血管瘤剥除术与介入栓塞治疗肝海绵状血管瘤的疗效比较研究

Comparative Study on Curative Effect of Hemangioma Stripping and Interventional Embolization for Hepatic Cavernous Hemangioma Patients

目的探讨血管瘤剥除术与介入栓塞治疗肝海绵状血管瘤的疗效及不良反应。方法收集2012年5月~2016年1月期间在本院接受手术治疗的肝海绵状血管瘤患者76例,根据手术方式将其分为:接受血管瘤剥除术治疗的对照组40例、接受介入栓塞治疗的观察组36例。评价两组患者的疗效、血清肝功能指标水平及不良反应。结果术后3月,两组患者的疗效比较,差异无统计学意义(P>0.05);术后1周,观察组外周血中肝功能指标ALT、AST、TBIL水平低于对照组,ALB水平高于对照组,差异均有统计学意义(P<0.05);术后3月,观察组发热、肝脓肿、胆管狭窄、肝功能不全等不良反应发生率显著低于对照组(P<0.05)。结论血管瘤剥除术与介入栓塞术均可有效治疗肝海绵状血管瘤,介入栓塞术对肝功能的损伤较小、术后不良反应发生率较低,安全性更好。

Objective To study the effect of hemangioma stripping and interventional embolization and their adverse reactions on he- patic cavernous hemangioma patients. Methods A total of 76 hepatic cavernous hemangioma patients were chosen as research subjects in our hospital during May 2012 and January 2016. Operation method was reviewed and all patients were divided into control group （n =40） in which patients under went hemangioma stripping, and observation group （n =36） in which patients underwent interventional emboliza- tion. After treatment, the curative effect, serum levels of liver function indices and adverse reaction of the two groups were analyzed. Results Three months after treatment, there was no significant difference in efficacy between the two groups （P〉0.05）. One week after treatment, liver function indices such as ALT, AST, TBIL in the observation group were lower than in the control group, while the ALB level in the ob- servation group was higher than in the control group （P〈0.05）. Three months after operation, the incidences of adverse reactions such as fe- ver, liver abscess, bile duct stricture and hepatic dysfunction in the observation group were lower than in the control group （P 〈 0.05）. Con- clusion Hemangioma stripping and interventional embolization can effectively treat hepatic cavernous hemangioma. There was less hepatic injury in interventional embolization and lower incidence of adverse reaction.
