T细胞亚群及其细胞因子与原发性肝癌及其预后的相关性分析

Correlation Analysis between T Lymphocyte Subsets together with its Cytokines and the Clinical Prognosis of Patients with Primary Hepatic Carcinoma

目的探讨T细胞亚群及其细胞因子与原发性肝癌预后的相关性。方法选择我院2012年1月~2013年6月收治的原发性肝癌患者80例作为研究组,同时选择80例健康成人作为对照组,检测两组外周血T细胞亚群(CD4^+、CD8^+T细胞)、CD4^+/CD8^+比值及相关细胞因子(IL-2、IL-6、IL-10、TNF-α)水平,并分析上述指标与肝癌患者3年生存率之间的关系。结果与对照组比较,研究组患者CD4^+T细胞水平显著降低(P<0.05),CD4^+/CD8^+比值显著下降(P<0.05);IL-2、IL-6、TNF-α水平显著降低,IL-10水平显著升高,差异均有统计学意义(P<0.05)。与存活患者比较,3年内死亡的患者CD4^+T细胞显著降低(P<0.05);IL-2、IL-6、TNF-α水平显著降低,IL-10水平显著升高,差异均有统计学意义(P<0.05)。结论原发性肝癌患者呈免疫抑制状态,表现为CD4^+T细胞和CD4^+/CD8^+比值降低,可导致患者临床预后恶化。

Objective To investigate the correlation of T lymphocyte subsets together with its cytokines and its clinical outcome in pa- tients with primary hepatocellular carcinoma. Methods 80 primary liver cancer patients were collected in our hospital as the study group, while 80 healthy adults were enrolled as the control group from January 2012 to June 2013. The levels of peripheral blood T lymphocyte subsets （CD4^＋, CD8^＋ T cells and CD4^＋/CD8^＋ T cells） and its cytokines （IL-2, IL-6, IL-10, TNF-a） in both groups were detected. In addi- tion, the differences of the peripheral blood T lymphocyte subsets and its cytokines between survival and death after 3 years were studied as well. Results Compared with the control group, patients in the study group got significantly lower levels of CD4^＋ T cells （P〈0.05）, a lower level of proportion of CD4^＋/CD8^＋ T cells, IL-2, IL-6 and TNF-c＋, a higher level of IL-10, and the differences were statistically significant （P〈0.05）. When compared with the survivals, patients suffered from death got a significantly lower level of CD4^＋ T cells, IL-2, IL-6 and TNF-α, a higher level of IL-10, and the differences were statistically significant （P〈0.05）. Conclusion Immune suppression was found in the patients with primary liver cancer, evidenced by decreased expression of CD4^＋ T cells and CD4^＋/CD8^＋ T cell ratio, and it was correlated with the poor clinical prognosis.
