摘要 我国是肝癌大国,肝癌的年发病及死亡人数均占全球一半以上.很多患者在肝癌发展的不同时期会接受放射治疗.放射性肝损伤(radiation-induced liver damage,RILD)是由放射治疗导致的一种临床亚急性、慢性肝脏损伤,是胸部和上腹部肿瘤放疗及骨髓移植前预处理中最严重的并发症之一.RILD极大地影响了胸腹部肿瘤,特别是肝癌的放疗疗效.因此,本综述对RILD的病理、影像学特点、临床评估及防治作一归纳总结. Liver cancer is a common cancer in China,with new cases and deaths accounting for more than half of those in the world each year. Radiation therapy is confirmed to be effective in liver cancer and can be applied in different stages of the disease. However,radiation-induced liver damage(RILD) is a common adverse effect characterized as sub-acute or chronic liver damage, which i s one of the most severe toxicities of radiotherapy for abdominal and thoracic tumors or treatment before bone marrow transplantation. Therefore, RILD limits the use of radiotherapy, especially impairing the effi cacy of radiotherapy. This article reviews the pathological characteristics, imaging features, evaluation and prediction, and treatment of RILD.
