自身抗体与慢性肾衰竭老年患者临床相关性分析

Clinical correlation analysis of autoantibodies and clinical symptom of elderly patients with chronic renal failure

目的探讨自身抗体对慢性肾衰竭老年患者临床特征的影响,为临床诊疗提供参考。方法将90例慢性肾衰竭老年患者根据自体抗体检测结果分为自身抗体阳性组46例和自身抗体阴性组44例,比较2组临床资料及实验室检查结果。结果阳性组局部感染和肾性骨病比例低于阴性组(P<0.05),部分器官切除术后比例高于阴性组(P<0.05)。Logistic回归分析结果显示,部分器官切除术后是产生自身抗体的独立危险因素(P<0.05),局部感染和肾性骨病是产生自身抗体的独立保护因素(P<0.05)。结论自身抗体对慢性肾衰竭老年患者临床症状有一定的影响,临床治疗中应给予重视。

Objective To understand the influence of autoantibodies on the clinical features of elderly patients with chronic renal failure, and to provide reference for clinical diagnosis and treatment. Methods Ninety elderly patients with chronic renal failure were divided into 2 groups according to the detection results of autoantibodies, including 46 cases of patients with autoantibodies（positive group） and 44 cases without autoantibody（negative group）. The clinical data and laboratory results of the 2 groups were compared. Results The proportion of local infections and renal osteodystrophy in positive group was lower than that in negative group（P〈0.05）. The proportion of partial organ resection was higher than that of negative group（P〈0.05）. Logistic regression analysis revealed that partial organ resection was a independent risk factor of producing autoantibodies（P〈0.05）. Local infections and renal osteodystrophy were independent protective factors of producing autoantibodies （P〈0.05）. Conclusion Autoantibodies have a certain influence on the clinical symptoms of elderly patients with chronic renal failure, and attention should be paid in clinical treatment.
