肠内营养联合大柴胡汤加味对胃癌术后早期免疫功能及胃肠功能的影响

Effect of enteral nutrition combined with Dachaihu Decoction on the early immune function and gastrointestinal function of postoperative patients with gastric cancer

目的观察肠内营养联合大柴胡汤加味对胃癌术后早期免疫功能及胃肠功能的影响。方法将80例胃癌术后患者随机分为治疗组与对照组各40例,术后均给予常规肠内营养,治疗组在此基础上给予大柴胡汤加味胃管注入,每日200 mL,分早晚2次注入。2组疗程均为14 d。疗程结束后比较2组免疫功能及胃肠功能指标。结果 2组治疗后CD3^+、CD4^+水平及CD4^+/CD8^+均明显升高而CD8^+明显降低(P均<0.05),且治疗组上述指标变化较对照组更明显(P均<0.05);2组治疗后IgA、IgM、IgG水平均较治疗前明显升高(P均<0.05),且治疗组较对照组改善更明显(P均<0.05);治疗后治疗组GAS水平无明显变化(P>0.05)但对照组明显降低且显著低于治疗组(P均<0.05);治疗组患者排气时间、排便时间及胃管放置时间均明显短于对照组(P均<0.05),胃管总引流量明显少于对照组(P<0.05)。结论肠内营养联合大柴胡汤加味对胃癌术后患者胃肠功能及免疫功能有明显恢复作用,值得推广应用。

Objective It is to observe the effect of enteral nutrition combined with Dachaihu Decoction on the early immune function and gastrointestinal function of postoperative patients with gastric cancer. Methods Eighty cases of gastric cancer patients were randomly divided into treatment group and control group with 40 cases in each group, all patients were given routine enteral nutrition, on the basis of it, the patients in the treatment group were treated with Dachaihu Decoction 200 mL per day, injected two times in the morning and evening by stomach tube. The course of treatment in the two groups was 14 days. After treatment, the immune function and gastrointestinal function indexes of the two groups were compared. Results After treatment, the levels of CD3 ^＋ , CD4 ^＋ and CD4 ^＋ /CD8 ^＋ in the two groups were significantly higher while CD8 ^＋ levels were significantly lower than those before treatment （all F 〈 0. 05 ） , and the treatment group was more obvious than the control group （ F 〈 0. 05 ） ; the levels of IgA, IgM and IgG in the two groups were significantly higher than those before treatment （ all F 〈0.05） , and the treatment group was significantly better than the control group （F 〈0. 05） ; the level of GAS in the treatment group was not significantly changed （ F 〉 0. 05 ） , but the control group was significantly lower than before treatment and the treatment group （all F 〈0. 05） ; the exhaust time, defecation time and gastric tube placement time of the treatment group were significantly shorter than those of the control group （all F 〈0.05） , and the total drainage volume of the gastric tube was significantly less than that of the control group （ F 〈 0.05 ） . Conclusion Enteral nutrition combined with Dachaihu Decoction has obvious curative effect on the recovery of gastrointestinal function and immune function of postoperative gastric cancer pa-tients ,it is worthy of popularization and application.
