丹红注射液对严重烧伤脓毒血症患者血浆IL-1β、MDA和SOD的影响及对急性肾损伤的保护效应 被引量:5

摘要 目的观察丹红注射液脓对严重烧伤脓毒血症患者血浆白细胞介素-1β(IL-1β)、超氧化物歧化酶(SOD)、丙二醛(MDA)水平的影响及对急性肾损伤的保护效应。方法将95例严重烧伤脓毒血症患者随机分为观察组48例和对照组47例,对照组给予西医常规治疗,观察组在对照组治疗基础上给予丹红注射液治疗,2组均连续治疗7 d。观察2组治疗前及治疗第3,5,7天的血浆IL-1β、MDA和SOD水平、血浆及尿液肾损伤分子-1(KIM-1)、胱抑素C(Cys C)、肾动脉收缩期峰值血流速度(Vp)、舒张期最小血流速度(Vd)、平均血流速度(Vm),血管阻力指数(RI)变化情况。结果 2组治疗后的血浆IL-1β、MDA水平均有一定升高,在治疗第3天达到高峰后降低,而SOD水平则呈现相反的变化趋势,观察组治疗第3,5,7天上述指标的改善情况均优于对照组(P均<0.05);2组治疗后的血浆及尿液KIM-1、Cys C水平均有一定升高,在治疗第3天达到高峰后降低,观察组治疗第3,5,7天上述指标均低于对照组(P均<0.05);2组在治疗第3,5,7天Vp、Vd、Vm均有提高,而RI降低,观察组治疗第3,5,7天的上述指标的改善情况均优于对照组(P均<0.05)。结论丹红注射液能改善严重烧伤脓毒血症患者肾功能,改善肾脏血流动力学,增加肾脏供血,具有显著的肾保护效应,其机制可能与调节血浆血浆IL-1β、MDA和SOD水平有关。 Objective It is to observe the effect of Danhong injection on plasma interleukin 1 beta ( IL-1β) , superoxide dismutase ( SOD) and malondialdehyde (MDA) and its protective effect of acute kidney injury in patients with severe burn sepsis. Methods 95 cases of severe burn patients with sepsis were randomly divided into observation group (48 cases) and control group (47 cases) , the control group was given conventional western medicine treatment, the observation group was giv-en Danhong injection on the basis of treatment in the control group, both groups were continuously treated for 7 d. The changes of plasma IL-1β, MDA and SOD, plasma and urine kidney injury molecules-1 urinary inhibition ( KIM-1),C (CysC),renal arter) systolic peak flow velocity (Yp) , minimum diastolic blood flow velocity (Yd) , mean blood flow velocity (Ym) , vascu-lar resistance index (RI) before treatment and at 3, 5,7 days after treatment were observed in both groups. Results After treatment plasma levels of IL-1β, MDA and plasma and urine levels of KIM-1, CysC were increased to some extent, and be-gan to be decreased after reaching its peak value in 3 days after treatment, but that of SOD showed a opposite changing trend, and the improvements of these indexes in 3 , 5,7 days after treatment in observation group were better than that in control group ( F 〈0. 05) . In 3 , 5, 7 days after treatment, Yp, Yd and Ym were increased while RI was decreased in both groups, and the improvements of these indexes in observation group were better than that in control group ( F 〈 0. 05 ) . Conclusion Danhong injection could improve renal function, improve renal hemodynamics, increase renal blood flow, has significant renal protective effect in patients with severe burn sepsis, its mechanism may be related to regulating blood plasma IL-1β, MDA and SOD levels.
