摘要
目的探讨钼靶X线检查对不典型乳腺癌的诊断价值。方法选择2014年8月—2016年4月期间深圳市中医院放射科接诊的40例不典型乳腺癌患者作为受试者,收集患者术前钼靶X线检查资料与术前或术中病理检查结果进行比对,并总结钼靶X线影响表现特征。结果病理检查结果显示:浸润性导管癌21例,粘液腺癌7例,髓样癌12例,与钼靶X线诊断结果比较,诊断符合率为87.50%(35/40)。40例患者的钼靶X线检查影像表现,发现21例浸润性导管癌,8例粘液腺癌均为边界清晰的肿块,可见粗乱索条影、高密度影,肿块结构杂乱,可见退化腺体中高密度结节影,伴有沙砾样钙化灶或细小钙化灶。8例髓样癌以边界清晰的肿块为主,其中4例患者则出现了致密结节边界不清晰影,部分患者致密腺体中可见肿块影或疑似肿块影。40例患者多为边界清晰的肿块,可见粗乱索条影、高密度影,肿块结构杂乱,退化腺体中高密度结节影,伴有沙砾样钙化灶。结论钼靶X线检查对不典型乳腺癌诊断准确性较高,应结合其影像表现特征性做出诊断,提高不典型乳腺癌检出率。
Objective To explore the diagnostic value of mammography in atypical breast cancer. Methods During August 2014 to April 2016 selected Shenzhen traditional Chinese medicine hospital radiology reception of 40 cases of atypical breast cancer patients as subjects to collect patients before and during the molybdenum target X- ray examination data and operation pathological examination results were compared, and summarizes the influence of feature of molybdenum target X-ray. Results pathological examination results showed that the invasive ductal carcinoma 21 cases, mucinous carcinoma in 7 cases, medullary carcinoma in 12 cases with molybdenum target X ray diagnosis results.The diagnosis coincidence rate was 87.5%(35 / 40), 40 patients with molybdenum target X- ray imaging performance,21 cases of invasive ductal carcinoma, 8 cases of mucinous adenocarcinoma were clear boundary mass, visible thick chaos bands, high density, mass structure clutter, visible degradation glands in high density nodules, accompanied by gravel like calcification or small calcification and 8 cases with medullary carcinoma with clear boundary mass, including 4 cases of patients with a dense nodules boundary is not clear Fuzzy shadow, with dense glands visible masses or suspected masses of 40 patients as masses with clear boundary, visible crude chaotic cable Shadow Stripe, high density,mass structure messy, degradation glands in the high density nodules, accompanied by gravel like calcification foci.Conclusion mammography examination of atypical breast cancer diagnostic accuracy should be combined with the imaging features of make a diagnosis, improve the detection rate of atypical breast cancer.
