应用微创经皮穿针复位技术治疗难复性股骨颈骨折

Application of Minimally Invasive Percutaneous Needle Assisted Reduction Technology in the Treatment of Irreducible Femoral Neck Fracture

目的探讨采用经皮穿针辅助复位技术在难复性股骨颈骨折治疗中的应用。方法回顾分析2012年4月—2014年3月应用微创经皮穿针复位技术治疗难复性股骨颈骨折23例的临床资料。结果 23例均获得闭合复位成功,参考Kyle等提出的标准并结合X线片对复位质量进行分级,优9例,良12例,中2例,优良率91.3%;2例骨折不愈合,骨折愈合率91.3%。结论采用经皮穿针辅助复位技术具有手术创伤小、复位效果好以及减少股骨头坏死发生率等优点,是治疗难复性股骨颈骨折值得推广的技术,大多数患者可获得满意效果。

Objective To explore the application of percutaneous needle assisted reduction technology in the treatment of irreducible femoral neck fracture. Methods By reviewing and analyzing 23 patients, admitted during March 2014 April 2012, who had femoral neck fracture and were treated with closed reduction and internal fixation. The patients were penetrated percutaneously with the Steinmann pin to the femoral head to assist reduction and underwent 3 cannu-lated screws fixation, due to difficulty in manual reduction. Results The group of 23 patients were all successful in closed reduction. They were graded on the quality of reduction according to the criterion put forward by Kyle et al. and the X-ray films. The results were excellent in 9 cases, good in 12, fair in 2, and the excellent and good rate was 91.3%. 2 cases were nonunion, and the fracture healing rate was 91.3%. Conclusion With the advantages of small sur-gical trauma, good effect of reduction and low rate of femoral head necrosis, percutaneous needle assisted reduction technology should be promoted in the treatment of irreducible femoral neck fracture. The majority of patients could achieve satisfactory results.
