经阴道彩色多普勒血流显像诊断盆腔静脉淤血症的临床研究

The Value of Transvaginal Color Doppler Flow Imaging in the Diagnosis of Pelvic Congestion Syndrome

目的分析经阴道彩色多普勒血流显像诊断盆腔静脉淤血症声像图特征,探讨彩色多普勒血流显像的诊断价值。方法回顾性分析该院2014年1月—2015年12月45例盆腔静脉淤血症患者经腹部超声和经阴道彩色多普勒血流显像征像,并与32例健康体检者进行比较。结果 45例盆腔静脉淤血症患者中轻度10例,中度25例,重度10例,轻、中、重度患者盆腔静脉内径均大于正常组,盆腔静脉血流速度小于正常组,差异有统计学意义(P<0.05)。轻、中、重度盆腔静脉淤血症患者盆腔静脉内径和血流速度比较差异均有统计学意义(P<0.05)。经阴道超声检出患者盆腔静脉曲张、子宫壁静脉曲张构成比例分别为99.5%和80.95%,检出率明显高于经腹超声,差异有统计学意义(P<0.05)。结论经阴道彩色多普勒血流显像简单、快捷、方便、无创,根据声像图特征、盆腔静脉内径和血流速度还可区分病情严重程度,具有较高的临床价值。

Objective To analysis the vaginal color Doppler flow imaging in diagnosis of pelvic vein congestion syn-drome. Methods A retrospective analysis of our hospital from January 2014 to December 2015. 45 cases of pelvic ve-nous stasis syndrome in patients with transabdominal ultrasound and transvaginal color Doppler flow imaging like syn-drome, and with 32 cases of healthy persons were compared. Results 45 cases of pelvic venous stasis syndrome in pa-tients with mild in 10 cases, moderate in 25 cases, 10 cases of severe, mild, moderate and severe patients with pelvic vein diameter were greater than the normal group, pelvic venous blood flow velocity is less than the normal group, the difference is statistically significant （P〈 0.05）. Mild, moderate and severe pelvic venous congestion syndrome patients with pelvic vein diameter and blood flow velocity differences were statistically significant （P〈0.05）. The proportion of patients with pelvic varicose veins and varicose veins were 97.78% and 82.22%, respectively, and the detection rate was significantly higher than that of abdominal ultrasound, the difference was statistically significant （P〈 0.05）. Con-clusion The transvaginal color Doppler flow imaging is simple, fast, convenient and noninvasive. According to the char-acteristics of the ultrasonic image, pelvic vein diameter and blood flow velocity can be distinguished by the severity of the disease, which has a high clinical value.
