Expression and Significance of MCP-1 in Elderly Patients with IgA Nephropathy

摘要 目的探讨单核细胞趋化蛋白-1(MCP-1)在老年IgA肾病(IgAN)患者肾脏组织、血液、尿液中的水平及意义。方法选取2014年1月—2016年1月在郑州颐和医院接受住院治疗的青少年、老年IgAN患者各30例,以及门诊体检健康的青少年、老年人各30名作为健康对照。根据所有老年IgAN患者的24 h尿蛋白定量值,将其分为不同的亚组;并根据Lee组织学分级系统将老年IgAN患者分为Ⅰ-Ⅴ级。采用免疫组织化学方法检测肾脏组织中MCP-1表达,ELISA法检测血液及尿液中MCP-1水平。结果老年IgAN患者肾脏组织、血液、尿液中的MCP-1表达水平显著高于青少年IgAN患者(P〈0.05):0.43%vs 0.40%、(124.32±20.12)pg/mL vs(116.34±16.37)pg/mL、(125.42±19.36)pg/mL vs(110.23±17.42)pg/mL。随着病情的加重,老年IgAN患者肾脏组织、血液及尿液中MCP-1表达水平也显著升高(P〈0.05)。结论老年IgAN患者肾脏组织、血液、尿液中的MCP-1表达水平显著增加,且与疾病进展有关,可作为辅助老年IgAN诊断及预后观察的潜在指标。 Objective This study mainly investigated the expression of monocyte chemoattractant protein(MCP)-1 in the kidney tissues, blood and urine samples from elderly patients with IgA nephropathy(IgAN), aiming to identify the role of MCP-1 in IgAN. Methods 30 cases of adolescents IgAN patients and 30 cases of elderly IgAN patients from January 2014 to January 2016 in Zhengzhou Yihe hospital were selected.30 cases of healthy adolescents and 30 cases of elderly healthy people as control from the Outpatient physical examination in the same hospital. The elderly patients with IgAN were divided into different subgroups according to the levels of 24 h urine protein excretion and Lee classification. The expression of MCP-1 in kidney tissues was analyzed by immunohistochemistry, while the levels of MCP-1in blood and urine samples were measured with an enzyme-linked immunosorbent assay(ELISA) kit. Results MCP-1expression in kidney tissues, or its levels in the blood and urine, from elderly patients with IgAN were significantly higher than those in young patients with IgAN(P〈0.05):0.43% vs 0.40%、(124.32±20.12)pg/mL vs(116.34±16.37)pg/mL、(125.42±19.36)pg/mL vs(110.23±17.42)pg/mL。 Moreover, in elderly patients with IgAN, the expression or levels of MCP-1 in kidney tissues or blood and urine samples were also increased with the higher(P〈0.05). Conclusion The expression levels of MCP-1 were significantly increased in elderly patients with IgAN, which was also closely related to the development of IgAN. This study indicated that MCP-1 might be one potential marker for the diagnosis and prognosis of IgAN in elderly patients.
