非静脉曲张性上消化道出血给予胃镜下注射肾上腺素并钛夹治疗的临床研究 被引量:7

Clinical Study on the Treatment of Non Varicose Upper Gastrointestinal Bleeding by Endoscopic Injection of Epinephrine and Titanium Clip

摘要 目的研究非静脉曲张性上消化道出血给予胃镜下注射肾上腺素并钛夹治疗的临床疗效。方法遴选2013年10月—2015年10月该院收治的84例非静脉曲张性上消化道出血患者为此次研究观察对象,均分为对照组与研究组各42例,前者仅在胃镜下注射肾上腺素治疗,后者同时还联用钛夹治疗,从临床诊断指标、止血情况等方面对两组的治疗效果进行统计分析。结果研究组的止血率为95.2%显著高于对照组的71.4%,而再次出血率仅为2.4%显著低于对照组的23.8%(P<0.05);研究组的出血量、输血量及住院时间(422.6±32.1)mL、(13.6±1.8)mL、(8.8±1.6)d,显著低于对照组的(626.8±42.4)mL、(26.3±2.3)mL、(11.6±2.3)d(P<0.05)。结论非静脉曲张性上消化道出血给予胃镜下注射肾上腺素并钛夹治疗止血效果显著,且能预防再次出血,促进患者的康复。 Objective To study the clinical effect of endoscopic injection of epinephrine and titanium clamp in the treatment of non varicose upper gastrointestinal bleeding. Methods We selected in our hospital from October 2013 to October 2015 84 cases in the non variceal upper gastrointestinal bleeding patients as the observation object, and divided into control group and study group, 42 cases in each, the former only in endoscopic injection of epinephrine in the treatment of, the latter also combined with titanium clips in the treatment of, from clinical diagnostic indexes and hemostasis etc. on the therapeutic effect of the two groups for statistical analysis. Results Study group of hemostatic rate was 95.2% significantly higher than that of the control group 71.4%, and re bleeding rate was only 2.4% was significantly lower than that of the control group of 23.8%(P〈0.05); study group of bleeding volume, blood transfusion and hospitalization time(422.6 ± 32.1) mL,(13.6 ± 1.8) mL,(8.8±1.6)d significantly lower than that of the control group(62.68 ± 4.24) mL,(26.3 ± 2.3) mL,(11.6 ± 2.3) d(P〈0.05). Conclusion Non varicose upper gastrointestinal bleeding treated by endoscopic injection of epinephrine and titanium clip in the treatment of bleeding, and can prevent bleeding again, promote the rehabilitation of patients.
