宫颈冷刀锥切和LEEP术对妊娠结局的影响

Effect of Cervical Cold Knife Conization and LEEP Surgery on Pregnancy Outcome

目的研究宫颈冷刀锥切术和LEEP术对妊娠结局的影响。方法:从该院2014年1月—2015年1月接收并行手术治疗的宫颈上皮内瘤变患者中,随机选取60例作为此组研究的对象,并按照不同的手术方式将其平均分为两组,一组为A组,另一组为B组,每组各有患者30例;A组应用宫颈冷刀锥切术治疗,B组采取LEEP术治疗,比较两种手术方式对患者妊娠结局的影响。结果 B组患者的胎膜早破、宫颈裂伤情况、平均孕周、低体重儿情况优于A组,且B组早产率为16.67%,显著优于A组的43.33%,差异具有统计学意义(P<0.05)。结论宫颈冷刀锥切手术会增加显著增加CIN患者术后早产风险,且对患者的胎膜早破、宫颈裂伤妊娠情况造成影响;采取宫颈环形电切术则较为安全,对患者的妊娠结局无显著影响,而且还能显著降低低体重儿的发生率,所以对保留生育功能的CIN患者,应优先选择LEEP术式进行治疗方。

Objective To study the effect of cervical cold knife incision and LEEP on pregnancy outcome. Methods Receive the parallel of surgical treatment of cervical intraepithelial neoplasia in patients from our hospital in January2014 to 2015 January, 60 cases were randomly selected as the study object and according to the different ways of operation will average into two groups, a group for group A, another group（group B）, each group has 30 cases of patients,a group of cervical cold knife conization cutting therapy, group B take LEEP treatment, compared two kinds of operation on patients with pregnancy outcome. Results B group of patients with premature rupture of membranes, cervical laceration, average gestational age, low weight infants were better than A group, and B group was 16.67%, significantly better than the A group of 43.33%, the difference was statistically significant（P〈0.05）. Conclusion The cervical cold knife cone cutting operation will increase significantly increased in patients with cervical intraepithelial neoplasia（CIN）after the risk of preterm birth and of patients with premature rupture of membranes, laceration of cervix pregnancy impact; take loop electrosurgical excision is safe and has no significant effect on patients with pregnancy outcome, but also significantly reduce the incidence of low birth weight infants, so to preserve reproductive function of patients with cervical intraepithelial neoplasia（CIN） should be preferred LEEP surgical treatment.
