1237例尿路感染细菌分布及耐药情况分析

Analysis of Bacterial Distribution and Drug Resistance of Urinary Tract Infection in 1237 Cases

目的分析郑州大学附属肿瘤医院2012—2015年1 237例尿路感染感染细菌分布及耐药情况,为临床合理使用抗菌药物提供科学依据。方法将2012年1月—2015年1月该院1 237例尿路感染标本分离出的细菌及药敏结果,应用EXCEL进行计算。按照细菌菌株种属差异进行分类,并对不同属菌株耐药性进行分析。结果该院尿路感染细菌监测送检标本共收集1 237例,主要为尿样,还有少量的造瘘管分泌物样本及导管尖端样本。培养出的G+菌株主要有葡萄球菌属、肠球菌属、链球菌属,体外实验对万古霉素敏感度高;G-菌株主要有埃希菌属、肠杆菌属、假单胞菌属、克雷伯菌属、不动杆菌属、变形杆菌属、哈夫尼亚菌属、克吕沃尔菌属、普罗威登斯菌属、无色杆菌属、枸橼酸杆菌属,体外实验对亚胺培南、含舒巴坦的复方制剂敏感度高;酵母样真菌属包括主要包括白色念珠菌、近平滑念珠菌、皮托念珠菌,对两性霉素B敏感。结论同一科不同种属的菌株对抗菌药物的敏感度差异较大,细菌耐药监测工作对临床合理使用抗菌药物非常重要,细菌分布及耐药监测情况分析对临床合理使用抗菌药物具有显著的指导意义。

Objective To provide scientific basis for clinical rational use of antimicrobial agents,the distribution and drug resistance of urinary tract infection bacteria in 1 237 patients with urinary tract infection in our hospital were analyzed. Methods The bacteria and drug sensitivity results of 1 237 cases of urinary tract infection in our hospital from January 2012 to January 2015 were analyzed by EXCEL. According to the difference of bacterial strains, the drug resistance was analyzed. Results Hospital urinary tract infection bacteria monitoring specimens 1 237 cases were collected, mainly for urine, and a small amount of made fistula secretion samples and catheter tip sample. Gram positive strains cultured mainly Staphylococcus genera and Enterococcus, Streptococcus, in vitro experiments were sensitive to vancomycin; gram negative strains are mainly Escherichia coli genera and Enterobacter spp, Pseudomonas, Klebsiella spp., Acinetobacter genera and Proteus, Hafnia genera, Kluyvera, Providencia, Achromobacter sp., Citrobacter spp., in vitro experiments of imipenem, containing high sensitivity of sulbactam compound preparation; yeast like fungi is mainly Candida albicans, Candida parapsilosis, pitot Candida, to amphotericin B sensitive. Conclusion In the same family,the sensitivity of the strains to antimicrobial agents is different, and the bacterial monitoring work is very important for clinical use.
