82株产酶大肠埃希菌的临床分布及耐药情况

摘要 目的了解产β-内酰胺酶大肠埃希菌在医院的检出情况和产酶菌株对药物的耐药性,为大肠埃希菌感染后的治疗提供合理的用药依据。方法收集临床分离的98株大肠埃希菌,细菌鉴定采用XIN-KE微生物鉴定仪;采用试纸法检测产酶株的阳性率;采用纸片法测定产酶阳性株对12种抗生素的耐药程度,分析其耐药性。结果98株大肠埃希菌中检出产酶菌株82株,阳性率为83.7%。产酶阳性株的标木来源以伤口分泌物为主,其次为尿液和痰液。科室以普外科、泌尿外科为主。产酶菌株对多种抗生素均表现出较强耐药性,其中对氨苄西林的耐药率最高为85.4%,对亚胺培南不耐药。结论临床分离大肠埃希菌产β-内酰胺酶检出率高,耐药率高,治疗产β-内酰胺酶菌感染应根据体外药敏试验结果选用敏感抗生素。 Objective To evaluate the distribution,rate of clinical detection,and antimicrobial resistance of producingβ-lactamase Escherichia coli to provide a basis for clinical treatment for producing β-lactamasr bacteria.Methods 98 strains of Escherichia coli were collected,the bacteria were identified by XIN-KE microbial identification instrument.The positive rate of producing strain was detected by test paper method.The antibiotic resistance of 12 strains of antibiotics was determined by paper method,and the drug resistance was analyzed.Results 98 clinical isolates of E.coli,there were 82 strains(83.7%) of producing β-lactamase E.coli.The sources of producing β-lactamase Escherrchia coli strains specimens were given priority to with the wound secretion,followed by urine and sputum.Strains were mostly from Ucneral Surgery and Urology.Producting β-lactamase E.coli had a high rate of antimicrobial resistance,there were high resistant(85.4%) to ampicillin,but sensitive to imipenem.Conclusion Producing β-lactamase E.coli was detected at a high rate in clinical isolates,and this producing β-lactamase E.coli is relatively resistant to antimicrobials.
