疝环充填式无张力修补术治疗腹股沟疝664例临床分析

Mesh plug tension-free herniorrhaphy for inguinal hernia:a clinical study of 664 cases

目的观察腹股沟疝疝环充填式无张力修补术的临床疗效。方法回顾性分析2012年2月至2015年12月,贵州省遵义医学院附属医院收治并完成疝环充填式无张力修补术的664例腹股沟疝患者的围手术期及随访资料,观察分析术后复发、术区不适及慢性疼痛、切口愈合不良、阴囊血清肿、尿潴留、附睾炎等情况,分析手术优劣并总结经验。结果 664例患者均顺利完成手术,手术时间30~75 min,平均(45±14)min;住院时间3~24 d,平均(9±4)d,术后随诊3~48个月,复发5例(0.75%),术区不适39例(5.9%),慢性疼痛7例(1.0%),脂肪液化38例(5.7%),切口浅层感染4例(0.6%),阴囊血清肿3例(0.5%),尿潴留52例(7.8%),附睾炎1例(0.2%)。结论疝环充填式无张力修补术治疗腹股沟疝疗效可靠、并发症少、易掌握,适宜基层医院广泛使用。

Objective To explore the clinical experience of mesh plug tension-free hernia repair.Methods From February 2012 to December 2015, a total of 664 cases of inguinal hernia, who were admitted to Affiliated Hospital of Zunyi Medical Colleague and treated with mesh plug herniorrhaphy, were retrospectively analyzed.Recurrence, postoperative inguinal discomfort and chronic pain, poor wound healing, scrotal seroma, urinary retention and epididymitis, etc., were observed and noted to sum up the experience of operation.Results All of the 664 patients were successfully operated, with the mean operating time of 45 minutes （35 to 70 minutes）, mean hospital stay of 6 days （3 to 24 days）.There were hernia recurrence in 5 cases, operation area discomfort in 39 cases, inguinal chronic pain in 7 cases, fat liquefaction in 38 cases, superficial wound infection in 4 cases, scrotal seroma in 3 cases, urinary retention in 52 cases, epididymitis in 1 cases.Conclusion Mesh plug tension-free hernia repair is a effective procedure with less complication and easy to master, which is suitable for popularization in primary hospitals.
