临床路径在预防剖宫产术后下肢深静脉血栓形成中的应用

Application of Clinical Pathway in Prevention of Deep Venous Thrombosis of Lower Extremities after Cesarean Sectio

目的:探讨临床路径在预防剖宫产术后下肢深静脉血栓形成(DVT)中的应用。方法:选取2016年3月至2017年2月阳西县妇幼保健院妇产科收治的剖宫产产妇200例,分为观察组和对照组两组,前者术后行临床路径方案;后者实施常规诊疗。记录并比较两组凝血功能指标、下肢肿胀率和DVT发生率。结果:护理前,两组的凝血指标比较,差异无统计学意义(P>0.05);护理后,两组均有所改善,观察组的凝血功能指标明显优于对照组,差异具有统计学意义(P<0.05)。通过对比,观察组的下肢肿胀率10.00%,DVT发生率为3.00%,明显低于对照组的25.00%和12.00%,差异具有统计学意义(P<0.05)。结论:对剖宫产产妇实施临床路径方案,更能有效降低凝血指标值,预防产妇下肢肿胀和DVT的形成。

Objective To explore the application of clinical pathways in the prevention of deep vein thrombosis（DVT） after cesarean section. Methods A total of 200 cases of cesarean section were collected from the Department of Obstetrics and Gynecology of Yangxi County Maternal and Child Health Hospital from March 2000 to February 2017. The patients were divided into two groups： observation group and control group. Treatment. The coagulation function, lower extremity swelling rate and DVT incidence were recorded and compared. Results Before nursing, the two groups of coagulant markers were statistically significant（P 〉0.05）. After nursing, both groups improved, and the observation group＇s coagulation index was significantly better than the control group, and the difference was statistically significant（P〈 0.05）. By contrast, observation group of lower limb swelling rate is 10.00 %, the incidence of DVT was 3.00 %, significantly lower than the control group were 25.00 % and 12.00 %, statistically significant difference（P〈 0.05）. Conclusions On the cesarean section of the implementation of clinical pathways program, more effective reduction of coagulation index values, prevention of maternal lower limb swelling and DVT formation.
