温阳益气方对慢性肾功能不全CKD2～3期的临床观察

Clinical Observation on the Effect of Wenyang Yiqi Decoction on CKD2～3 in Chronic Renal Insufficiency

目的观察温阳益气方对慢性肾功能不全CKD2~3期的临床疗效。方法选取2014年2月—2016年2月在本院住院的慢性肾功能不全CKD2~3期患者60例,随机分为对照组和治疗组,每组30例。对照组予西医常规治疗,治疗组在对照组治疗的基础上加用温阳益气方,治疗6个月,观察治疗前后2组患者的肾小球率过滤(eGFR)、血尿素氮(BUN)、血肌酐(Scr)、24 h尿蛋白定量、血浆白蛋白(Alb)等指标。结果 (1)2组临床疗效比较:治疗组显效12例,有效14例,总有效率86.7%;对照组显效8例,有效12例,总有效率66.7%。治疗组总有效率优于对照组(P<0.05)。(2)与治疗前比较,2组患者治疗后24 h尿蛋白定量均有不同程度提高(P<0.05);与对照组比较,治疗组各项肾小球率过滤(eGFR)、血尿素氮(BUN)、血肌酐(Scr)改善更显著(P<0.05)。(3)与治疗前比较,2组Scr均下降(P<0.05,P<0.01),GFR明显提高(P<0.05),治疗组疗效更显著(P<0.05)。结论温阳益气方能明显改善慢性肾功能不全CKD2~3期患者的肾功能。

Objective： To observe the clinical effect of Wenyang Yiqi Decoction on CKD2 -3 in chronic renal insufficiency. Methods： 60 chronic renal insufficiency patients who were hospitalized in our hospital from February 2014 to February 2016 were randomly divided in- to a control group and a treatment group, 30 cases per group. The control group was treated with conventional western medicine while the treatment group was added Wenyang Yiqi Decoction on the base of the conventional western medicine. After 6 - month treatment, each glo- merular filtration rate （eGFR）, blood urea nitrogen （BUN）, serum creatinine （Scr）, 24 -hour urinary protein quantification and plasma albumin （Alb） were measured before and after treatment. Results： （1） The clinical effect of the treatment group was that 12 cases were remarkably effective and 14 cases effective, and the total effective rate was 86. 7% , whereas the total effect of the control group was that 8 cases were remarkably effective, 12 cases effective, and the total effective rate was 66.7%. The total effective rate of the treatment group was better than that of the control group （P 〈 0.05）. （2） Compared with that of prior treatment, the level of 24 - hour urinary protein of the two groups increased to varying degree after treatment （P 〈0. 05, P 〈0. 01 ）. Compared with those of the control group, the levels of eGFR, BUN and Scr significantly improved （P 〈 0. 05）. （3） Compared with that of prior treatment, Scr of the two groups decreased （P 〈 0. 05 ） and GFR significantly increased. The curative effect of the treatment group was more significant （P 〈 0.05 ）. Conclusion ： Wenyang Yiqi Decoction can significantly improve the renal function of CKD2 -3 patients with chronic renal insufficiency.
