口腔冷疗法预防放疗联合化疗移植预处理患者口腔黏膜炎11例疗效观察

11 cases of oral cold therapy to prevent radiation combined chemotherapy transplantation pretreatment observation of oral mucositis in patients

目的:通过观察口腔冷疗法预防造血干细胞移植患者并发口腔黏膜炎的效果,探索适合放疗联合化疗作为移植预处理方案患者的口腔护理路径。方法:随机选择21例放疗联合化疗作为移植预处理进行造血干细胞移植的患者,在进行常规口腔护理的基础上,根据预处理期间是否使用口腔含冰水法,将研究对象分为实验组和对照组。对照组10例,实施常规两种漱口液含漱,即A液为生理盐水500 m L+庆大霉素注射液20万U+四氢叶酸钙25 mg,B液为生理盐水500 m L+制霉菌素5片。实验组11例,在实施常规A、B两种漱口液交替含漱口的基础上,分别在放疗及输注化疗药物开始前30 min至结束后30 min,每隔15~30 min口含冰水,每次2~3 min。比较两组发生口腔黏膜炎的程度及出仓时间。结果:实验组有效控制了预处理方案所致的3级以上口腔黏膜炎的发生,缩短了入住移植仓内的时间,与对照组比较,差异有统计学意义。结论:口腔冷疗法可降低放疗联合化疗作为造血干细胞移植预处理患者口腔黏膜炎的发生。

Objective： Through the mouth cold therapy to prevent hematopoietic stem cell transplantation patients complicated with oral mucositis effect observation,to explore radiotherapy combined chemotherapy pretreatment scheme as a transplant patients oral care path. Methods： Randomly selected from 21 cases radiotherapy combined chemotherapy as a pretreatment for hematopoietic stem cell transplantation patients,on the basis of conventional oral care,according to whether to use the mouth during pretreatment including water method,the object of study is divided into the experimental group and control group.10 cases of control group,the implementation of the conventional two mouthwash contain gargle,namely A liquid： physiological saline 500 mL ＋ 200 000 U gentamycin sulfate injection 25 mg calcium,folic acid liquid B： physiological saline 500 mL ＋ 5 pieces of nystatin. Experimental group in 11 cases,in the implementation of regular contains A and B two alternating mouthwash mouthwash,on the basis of respectively in 30 minutes before the infusion of radiotherapy and chemotherapy drugs to after 30 minutes,15 - 30 minutes every mouth with ice water,2 - 3 minutes at a time. Compared two groups of the degree of inflammation of the oral mucosa and out of the storehouse time comparison. Results： The effective pretreatment control scheme of the experimental group of more than 3 oral mucositis,short the time of check in transplantation in,have obvious differences compared with control group. Conclusion： Oral cold therapy can reduce radiation combined chemotherapy as a pretreatment of oral cavity mucous membrane inflammation in patients with hematopoietic stem cell transplantation.
