孕早期血红蛋白在妊娠期糖尿病发病中的重要性 被引量:16

摘要 目的:分析孕早期血红蛋白(hemoglobin,Hb)与妊娠期糖尿病(gestational diabetes mellitus,GDM)发生的相关性。方法:单胎妊娠孕妇872例在孕12~14周检测Hb含量,24~28周行75 g口服葡萄糖耐量试验(OGTT)筛查GDM,进行GDM患者Hb水平与GDM发生的相关性分析,建立GDM预测模型等。结果:经过独立样本t检验,年龄、孕期体重增加量、孕期体重指数(BMI)增加量、Hb、血小板计数(PLT)、红细胞计数(RBC)、红细胞压积(HCT)可能为GDM的影响因素(P<0.05)。Logistic回归分析结果显示,年龄、孕期BMI增加量、妊娠14周前Hb、RBC、HCT为妊娠晚期GDM发生的独立影响因素(P<0.05)。进一步行受试者工作特征(ROC)曲线分析显示,可将孕早期Hb 126.5 g/L作为预测GDM的临界值。结论:孕早期Hb水平与妊娠糖尿病呈现正相关关系,并且早期应该控制Hb水平在126.5 g/L以下,降低GDM发生风险。
