进展期胃癌患者病理特征与预后相关因素分析

Pathological characteristics and prognostic factors of advanced gastric cancer

目的分析进展期胃癌患者病理特征与预后相关因素。方法选取该院2013年3月至2016年9月收治的进展期胃癌患者37例作为研究对象,结合患者一般资料、病理特征及治疗过程与结果进行回顾性分析。结果单因素分析显示,肿瘤部位、大小、类型、浸润深度及淋巴结转移与进展期胃癌患者预后紧密相关。多因素分析显示,进展期胃癌患者预后的危险因素可通过淋巴结转移进行提示。37例患者均进行了手术治疗,术后5年生存率为56.76%(21/37);1~3枚淋巴结转移患者术后5年生存率为29.73%(11/37),≥4枚淋巴结转移患者术后5年生存率为16.22%(6/37)。结论在影响进展期胃癌患者的预后因素中,淋巴结转移为进展期胃癌的重要影响指标,在术前对淋巴结转移进行评估,有利于制订合理的治疗方案,临床推广应用价值较高。

Objective To analyze the pathological features and prognostic factors of advanced gastric cancer. Methods A total of 37 patients with advanced gastric cancer treated in our hospital from March 2013 to September 2016 were selected as subjects. The basic data, pathological features,treatment course and results were retrospectively analyzed. Results Single factor analysis showed that tumor size,depth of invasion and lymph node metastasis were closely related to the prognosis of patients with advanced gastric cancer. Multivariate analysis showed that the risk factors of prognosis in advanced gastric cancer patients could be prompted by lymph node metastasis. All the 37 patients underwent surgical treatment of which the postoperative 5 year survival rate was 56.76%（21/37）;patients with 1-3 lymph nodes metastasis postoperative 5 years survival rate was 29.73%（11/37） and patients with 4 or more lymph nodes metastasis postoperative 5 year survival rate was 16.22%（6/37）. Conclusion Among the factors affact prognosis,lymph node metastasis is an important index in patients with advanced gastric cancer. Lymph node metastasis assessment before the preoperation is conducive to the development of rational treatment plan,and it has higher values in clinical application.
