术前超声联合Mammotome Elite系统对乳腺癌腋窝淋巴结评估意义

Significance of Assessing Axillary Lymph Nodes Status in Breast Cancer by Preoperative Ultrasonography Combined with Mammotome Elite System

目的评估术前超声引导下Mammotome Elite系统对乳腺癌腋窝淋巴结的评估意义。方法收集自2015年5月至2017年3月期间,我院64例确诊乳腺癌并怀疑腋窝淋巴结转移的患者,其中Elite组入组32例,在超声引导下采用Mammotome Elite系统进行淋巴结活检;粗针穿刺组入组32例,在超声引导下行腋窝淋巴结穿刺活检。最终所有患者均行腋窝淋巴结清扫术,以评估淋巴结穿刺活检的准确性。结果 Mammotome Elite系统术前诊断腋窝淋巴结转移的灵敏度、特异度和符合率分别为89.7%、100%和90.6%,粗针活检的灵敏度、特异度和符合率分别为76.9%、100%和76.5%,且ROC曲线下面积前者(0.946)高于后者(0.885)。结论 Mamotone Elite系统对乳腺癌腋窝阳性淋巴结的检出功效较粗针活检好。

Objective To assess the significance of preoperative uhrasonography combined with Mammotome Elite system to theaxillary lymph nodes status in breast cancer. Methods 64 patients, who were confirmed with breast cancer and suspected with axilla-ry lymph nodes metastasis in our hospital from May 2015 to March 2017 were selected. 32 cases in the Elite group were given Mammo-tome Elite biopsy guided by ultrasonography for axillary lymph nodes biopsy. 32 cases in the Bodkin Puncture group were given punc-ture guided by ultrasonography for axillary lymph nodes biopsy. Finally, the axillary lymph nodes dissection was performed on all pa-tients to assess the accuracy of lymph nodes puncture biopsy. Results The sensitivity, specificity, coincidence rates of MammotomeElite system and bodkin puncture biopsy guided by preoperative uhrasonography to assess axillary lymph nodes status were 89.7% ,100%, 90. 6% and 76. 9%, 100% , 76. 5% respectively. And the area under the ROC curve of Mammotome Elite （0. 946） washigher than that of the puncture biopsy （0. 885 ）. Conclusion Mammotome Elite has a better efficacy to detect postive axillary lymphnodes in breast cancer than bodkin biopsy.
