输尿管镜下钬激光碎石术治疗输尿管结石的临床疗效分析

Clinical Efficacy of Holmium Laser Lithotripsy Under Ureteroscopy on Ureteral Calculi

目的探讨输尿管镜下钬激光碎石术治疗输尿管结石的临床疗效。方法分析我院应用手术方法治疗输尿管结石50例患者的临床资料,其中输尿管上段结石8例,中段结石18例,下段结石24例。结果输尿管镜下钬激光碎石术治疗输尿管结石25例,有2例输尿管上段结石碎石后残石进入肾盂,其余均碎石成功。与传统开放手术相比,输尿管镜下钬激光碎石术具有术中出血、手术时间、术后住院时间、双J管放置时间均明显缩短,术中术后并发症明显减少,患者满意度明显增加,临床疗效增加。传统输尿管切开取石术治疗输尿管结石25例,均成功取出结石,但术中出血、手术时间、术后住院时间、双J管放置时间均明显增高,术中术后并发症明显增加,患者满意度明显降低,临床疗效降低。结论输尿管镜下钬激光碎石术是一种安全、有效、可重复进行的治疗输尿管结石的好方法,适合在广大医院大力推广。

Objective To investigate the clinical efficacy of holmium laser lithotripsy under ureteroscopy on ureteral calculi.Methods Analysis was made of 50 cases of ureteral calculi who received operation in our hospital. Of the 50 cases, 8 cases sufferedfrom the upper ureteral calculi and 18 cases suffered from the middle part, while 24 cases suffered from the distal part. Results Re-sidual ureteral calculus in the 2 cases among the 25 cases received holmium laser lithotripsy under ureteroscopy entered into the renalpelvis after lithotripsy while the other cases were treated successfully. Compared with traditional open operation, holmium laser litho-tripsy under ureteroscopy had such advantages as less intraoperative bleeding, and shortened operation time, postoperative hospitalstay and double J tube placed time. As intraoperative and postoperative complication decreased, patient satisfaction and clinical cura-tive effect increased. Stones were removed successfully in 25 cases received traditional ureteral incision nephrolithotomy, but with moreintraoperative bleeding, longer operation time, postoperative hospital stay and double J tube placed time. With increased intraopera-tire and postoperative complications, patient satisfaction and clinical efficacy significantly declined. Conclusion Holmium laser lith-otripsy under ureteroscopy is a safe, effective, reliable method in the treatment of ureteral calculi, and worthy of promotion.
