负压封闭引流术在腕、掌部开放性骨折合并软组织缺损病例中应用

Application of Vacuum Sealing Drainage in Cases of Open Fracture of Wrist or Metacarpus Combined with Soft Tissue Defect

目的观察腕、掌开放性骨折合并软组织缺损病例治疗中负压封闭引流术的治疗效果。方法回顾我科2012年10月至2014年10月收治的腕、掌部开放性骨折合并软组织缺损并行负压封闭引流治疗的82例患者的临床资料,分析负压封闭引流术的治疗效果。结果入选病例I期均经负压封闭引流治疗,Ⅱ期行游离植皮57例,皮瓣移植25例,所有患者植皮或皮瓣移植均成活。术后定期随访,所有患者腕、掌关节功能均恢复满意。结论腕、掌部开放性骨折合并软组织缺损的病例,通过负压封闭引流术的应用,修复腕、掌部创面,改善伤肢关节功能,提升治疗效果明显。

Objective To observe the therapeutic effect of vacuum sealing drainage in the cases of open fracture of wrist or meta-carpus combined with soft tissue defect. Methods Clinical data of 82 cases of open fracture of wrist or metacarpus combined with softtissue defect treated by vacuum sealing drainage from October 2012 to October 2014 were reviewed and the therapeutic effects of vacu-um sealing drainage were analyzed. Results All the selected cases were treated by vacuum sealing drainage in the first stage. In thesecond stage, there were 57 free skin graft cases and 25 flap transplantation cases. All the cases with free skin graft or flap transplanta-tion survived. Regular follow-up showed that all the patients with wrist and metacarpal joint function were satisfied with the recovery.Conclusion The application of vacuum sealing drainage in the cases of open fracture of wrist or metacarpus combined with soft tissuedefect may repair the wound of wrist and metacarpal, improve the function of injured joint and significantly enhance the therapeuticeffect.
