儿童陈旧孟氏骨折的治疗进展

Progress in therapies of aged Monteggia fracture in children

儿童孟氏骨折由于各种原因的漏诊、误诊常发展为陈旧骨折，严重影响肘关节生长发育。目前对陈旧孟氏骨折的研究报道很多，治疗方法多样，但尚缺少规范化的治疗标准。本文通过检索国内外文献，对儿童陈旧孟氏骨折的手术治疗及术后并发症作一综述。尺骨截骨联合切开复位以及根据术中情况决定是否实施环状韧带重建是目前广泛采用并且有效的手术治疗措施，术后并发症主要为桡骨头半脱位、关节活动受限。

Monteggia fracture often develops into aged Monteggia fracture due to various causes of neglected or missed diagnosis. And it affects seriously the growth and development of elbow joint. Therapy of aged Monteggia fracture has been extensively reported. However, there is a lack of specific guidelines. This systematic review offers an overview of surgical managements and postoperative complications of aged Monteggia fracture in children. Ulnar osteotomy plus open reduction with or without reconstruction of annular ligament is effective and widely applied in clinical practice. Radial head subluxation and joint motion limitation are two major postoperative complications.
