妊娠期高血压疾病住院患者用药分析

Analysis of uses drugs in inpatients with hypertesive disorders of pregnancy

目的回顾性分析妊娠期高血压疾病住院患者的用药情况,分析其用药合理性。方法收集该医院产科2011年1月至2015年6月主要诊断为妊娠期高血压疾病的住院患者基本资料、用药信息和费用信息,用SPSS22.0软件对数据进行统计分析。结果共纳入妊娠期高血压疾病住院患者974例,其中,重度子痫前期患者最多,占38.81%(378/974例)。对使用药物进行分析,共包括降压药物4种、防治子痫药物3种、镇静药物3种、利尿剂、纠正低蛋白血症药物和中成药各1种。各亚组用药情况略有差异。使用频次前10位的药品为维生素C、维生素E、硝苯地平、川芎嗪、硫酸镁、硝苯地平缓释片、白蛋白、地西泮片、乌拉地尔和呋塞米。结论妊娠期高血压疾病住院患者的药物治疗是以防治子痫前期和降压为主,同时中成药川芎嗪联合使用,符合疾病诊疗指南用药要求,用药基本合理。但维生素C和维生素E的使用存在争议。

Objective To retrospectively investigate the drug use of the patients with hypertensive disorders of pregnancy （HDP） so as to eva- luate the rationality of the drug use among these patients. Methods The information of the inpatients with HDP as the initial diagnosis in this hos- pital from January 2011 to June 2015 was collected, including the patient characteristics, drug use, costs, etc. The data was analyzed by the SPSS 22.0 software. Results In sum, 974 HDP cases were included, among which severe preeclampsia is the main component with 38. 81% （378/ 974 cases） . Drug analysis showed that there were four types of anti - hypertensive drugs, three preeclampsia - prophylaxis drugs, three sedatives, one diuretic, one anti -hypoalbuminemia drug, and one Chinese medication. The patients were divided into subgroups according to HDP classification and there was subtle difference among the subgroups. The top ten drugs according to the use frequency were vitamin C, vitamin E, nifedipine, ligustrazine, magnesium sulfate, nifediping sustained - release tablets, albumin, diazepam, urapidil, and furosemide. Conclusion The HDP treatments were mainly composed by the preeclampsia prophylaxis and the anti - hypertension. Ligustrazine acted as an adjunctive treatment. The treatments above were consistent with the guidelines, which was rational. But the use of Vitamin C and E was controversial.
