以髋关节疼痛为主的非典型腰椎间盘突出症的诊治

Diagnosis and Treatment of Hip Pain as the Main Performance of Atypical LIDH

目的探讨非典型腰椎间盘突出症的发病原因、诊断及治疗,提高临床医生的关注度,减少误诊或漏诊的发生。方法回顾性分析我院门诊2015年12月—2016年6月收治的以髋关节疼痛为主要表现的20例患者的临床资料。结果所有患者均行腰部CT或MRI检查证实存在腰椎间盘多节段膨出或突出,采取非手术方式治疗均取得明显效果。结论对于临床无明显急性腰部外伤史,仅表现为髋关节疼痛的患者(排除髋部疾病),可及早行腰部CT或MRI检查以明确诊断,对确诊为腰椎间盘突出症患者采取非手术治疗方式可取得显著疗效。

Objective To investigate the etiology,diagnosis and treatment of atypical lumbar disc herniation,to improve the concern of clinicians,and to reduce the incidence of misdiagnosis or missed diagnosis. Methods The clinical data of 20 patients with hip pain as the main manifestation were analyzed retrospectively from December 2015 to June 2016 in our hospital. Results All patients underwent lumbar CT or MRI examination confirmed the existence of multi-segmental prolapse or prominent lumbar intervertebral disc,taking non-surgical treatment have achieved significant results. Conclusion There is no obvious clinical history of acute lumbar trauma in patients with hip pain（ excluding hip disease）. Early diagnosis of lumbar CT or MRI can be used to diagnose patients with lumbar disc herniation without nonsurgical treatment way to obtain a significant effect.
