早期治疗性ERCP治疗急性胆源性胰腺炎的疗效分析

Efficacy analysis of early therapeutic ERCP in the treatment of acute biliary pancreatitis

目的 探讨急性胆源性胰腺炎早期行治疗性内镜逆行性胰胆管造影术(ERCP)治疗的临床疗效。方法将96例胆源性胰腺炎患者随机分为内镜治疗组51例和保守治疗组45例。内镜治疗组在保守治疗的基础上,早期予以经内镜逆行性胰胆管造影术(ERCP)、内镜下乳头括约肌切开术(EST)/内镜下气囊扩张术(EPBD)/经内镜鼻胆管引流术(ENBD)治疗,保守治疗组采用药物保守治疗,比较两组疗效。结果内镜治疗组患者在腹部疼痛好转时间、血淀粉酶、肝功能及CRP(C反应蛋白)恢复时间、住院时间方面均有所减少,两组比较,差异有统计学意义(P<0.05)。结论急性胆源性胰腺炎早期行治疗性内镜逆行性胰胆管造影术治疗安全、有效,创伤小、恢复快、治愈率高、并发症低,安全有效,值得临床推广。

Objective To explore the clinical efficacy of early endoscopic retrograde cholangiopancreatography （ERCP） in the treatment of acute biliary pancreatitis. Methods A total of 96 patients with biliary pancreatitis were randomly di- vided into endoscopic treatment group （n=51） and conservative treatment group （n=45）. The endoscopic treatment group was treated with endoscopic retrograde cholangiopancreatography （ERCP）, endoscopic sphincterotomy （EST）/endoscopic balloon dilatation （EPBD）/endoseopic nasobiliary drainage （ENBD） treatment at the early stage on the basis of conservative treatment. And the conservative treatment group was given conservative treatment of drugs. The efficacy between the two groups was compared. Results The abdominal pain improved time, blood amylase, liver function and CRP （C-reac- tive protein） recovery time, length of stay were reduced in the endoscopic treatment group compared with those in the conservative treatment group. And the difference between the two groups was statistically significant（P〈O.05）. Conclu- sion Endoscopic retrograde cholangiopancreatography in the treatment of acute biliary pancreatitis is safe, effective with less trauma, quick recovery, high cure rate, low complication, and is worthy of clinical promotion.
