大隐静脉高位结扎术联合泡沫硬化剂注射治疗下肢静脉曲张的临床疗效探讨

Discussion on the clinical effect of high ligation of great saphenous vein combined with sclerosing foam injection in the treatment of varicose veins of lower extremities

目的观察大隐静脉高位结扎术联合泡沫硬化剂注射治疗下肢静脉曲张的临床疗效。方法选取我院104例下肢静脉曲张患者为研究对象,随机分为观察组与对照组,每组各52例,观察组采取大隐静脉高位结扎术联合泡沫硬化剂注射治疗,对照组采取传统大隐静脉高位结扎剥脱术治疗,比较两组患者术中、术后主要观察指标、临床症状、体征改善情况、临床疗效、并发症发生率、术后生活质量等。结果观察组手术时间、术中出血量、住院时间等指标明显低于对照组;观察组临床治疗总有效率、生活质量明显高于对照组,差异有统计学意义(P<0.05);两组患者临床症状、体征改善情况、并发症发生率比较差异无统计学意义(P>0.05)。结论大隐静脉高位结扎术联合泡沫硬化剂注射治疗下肢静脉曲张与传统手术比较,具有创伤小、疗效高、复发率低等优点,且患者满意度以及术后生活质量高,值得临床推广。

Objective To observe the clinical effect of high ligation of great saphenous vein combined with sclerosing foam injection in the treatment of varicose veins of lower extremities. Methods 104 patients with varicose veins of low- er extremities in our hospital were selected as the study objects. The patients were randomly divided into observation group and control group, with 52 patients in each group. The observation group was given high ligation of great saphe- nous vein combined with sclerosing foam injection,and the control group was given conventional high ligation and strip- ping of great saphenous vein. The main intraoperative and postoperative observation indices,improvement of clinical symptoms and signs, clinical curative effect,incidence rate of complications and postoperative quality of life were com- pared between the two groups. Results The surgery time, intraoperative blood loss and length of stay in the observation group were significantly lower than those in the control group; the total effective rate of clinical treatment and quality of life in the observation group were significantly higher than those in the control group, the differences were statistically significant（P〈0.05）; there was no statistically significant difference in the improvement of clinical symptoms and signs and incidence rate of complications between the two groups（P〉0.05）. Conclusion High ligation of great saphenous vein combined with sclerosing foam injection in the treatment of varicose veins of lower extremities has the advantages of less trauma,high curative effect and low recurrence rate compared with traditional surgery,and patients＂ satisfaction and postoperative quality of life are high, which is worthy of clinical promotion.
