乳腺癌患者早期保乳手术联合辅助化疗的临床效果评价

Clinical effect evaluation of early breast conserving surgery combined with adjuvant chemotherapy in patients with breast cancer

目的探讨乳腺癌患者早期保乳手术联合辅助化疗的临床效果及安全性。方法选择2014年8月~2015年8月我院收治的90例早期乳腺癌患者,将其随机分为观察组和对照组,每组45例。对照组患者采用早期乳腺癌根治术,观察组患者给予早期保乳手术治疗,两组术后均辅以化疗。术后随访1年,记录两组患者的美容效果及生活质量,同时比较两组的并发症发生率及复发率。结果与对照组比较,观察组的手术时间及住院时间均显著减短,术中出血量、引流量显著较少,差异有统计学意义(P<0.05)。观察组患者的术后美容效果优良率(93.33%)显著高于对照组(66.67%),差异有统计学意义(P<0.05)。两组的并发症发生率及局部复发率比较差异无统计学意义(P>0.05)。观察组的生活质量评分为(96.1±4.2)分,显著高于对照组的(78.9±5.3)分,差异有统计学意义(P<0.05)。结论早期保乳手术联合辅助化疗治疗早期乳腺癌可有效提高患者术后乳房美观度,保障患者的生活质量,且并发症发生率和局部复发率较低,值得临床推广应用。

Objective To investigate the clinical effect and safety of breast conserving surgery combined with chemotherapy in patients with breast cancer.Methods 90 cases with early breast cancer treated in our hosp August 2014 to August 2015 were selected and they were randomly divided into the observation group and th adjuvant ital from e control group,and each group had 45 cases.Patients in the control group were treated with early breast cancer radical operation, while the observation group was given early breast conserving surgery,and the two groups were treated with chemothera- py after operation.All patients were followed up for one year.The cosmetic effect and quality of life of the two groups were recorded,and the incidence of complications and recurrence rate were compared in patients between the two groups.Results Compared with the control group,observation group,operation time and hospitalization time were signifi- cantly shorter, intraoperative blood loss and volume of drainage were significantly less in the observation group,and the difference was statistically significant （P〈0.05）.The good and excellent rate of cosmetic effect after operation in the ob- servation group （93.33%） was significantly higher than that of the control group （66.67%）,and the difference was statis- tically significant （P〈0.05）.There was no statistical difference of the incidence of complication and local recurrence rate between the two groups （P〉0.05）.Quality of life score of the observation group [（96.1±4.2） points] was significantly high- er than that of the control group [（78.9±5.3） points]s,and the difference was statistically significant （P〈0.05）.Conclusion Early breast conserving surgery combined with adjuvant chemotherapy treating early breast cancer can effectively im- prove the degree of breast appearance after surgery of patients,protect the quality of life of patients,and the incidence of complications and the recurrence rate is low,it is worthy of clinical promotion and application.
