摘要 2010年,Barry和Vaezi[1]总结了文献对咽喉反流（laryngopharyngeal reflux,LPR）的共同描述：患者主诉慢性声音嘶哑、清嗓、咳嗽、吞咽困难、咽喉异物感等症状,进行喉镜检查排除癌症,诊断为LPR,质子泵抑制剂（proton pump inhibitor,PPI）往往规定每天2次,持续2个月.他们提出：（1）喉镜检查因评分者间差异大,pH监测结果不可靠,两者均不能预测患者的治疗效果;（2）如果患者喉部症状和体征表明每天2次且持续2个月的PPI治疗有效,逐渐减量到每天1次,然后以最低剂量的酸抑制来控制症状是审慎的;（3）PPI治疗无效的患者不太可能从手术中获益,要考虑如过敏性疾病、鼻窦炎性反应及肺部疾病等的可能,同时逐步减少药物剂量以防止酸反流的反弹.由于缺乏随机、安慰剂对照试验的确凿证据,LPR的诊断和治疗仍有争议.
