四川地区糖尿病、糖耐量减退人群血流变、凝血指标相关性分析 被引量:3

摘要 目的 通过回顾性分析探讨健康体检人群中糖尿病及糖耐量减低患者血流变、凝血等指标的变化情况.方法 选取2015年1月—2016年10月在四川省人民医院健康体检中心体检人员共2,195人,分为糖尿病组、糖耐量减低组及对照组,抽取静脉血测定空腹血糖、糖化血红蛋白、血流变、凝血等指标,比较各组之间的差异.结果 糖尿病组、糖耐量减低组全血黏度、血浆黏度、红细胞刚性指数、红细胞聚集指数显著高于对照组,且与糖化血红蛋白水平呈正相关;红细胞变形指数低于对照组,与糖化血红蛋白水平呈负相关(P〈0.05).同时糖尿病组、糖耐量减低组的凝血酶原时间、活化部分凝血活酶时间、凝血酶时间也显著低于对照组,血浆纤维蛋白原浓度高于对照组(P〈0.05).结论 糖尿病患者血流变、凝血情况出现异常,处于相对高凝状态,可能对糖尿病慢性血管病变发生发展起到促进作用. Objective To retrospectively analyze the changes of hemorheological indexes and blood coagulation of the people who suffered from diabetes mellitus (DM) and impaired glucose tolerance (IGT). MethodsWe have 2,195 persons from January 2015 to October 2016 who have a health checkup in the health examination center of Sichuan Provincial People's Hospital in this study, to measure the fasting glucose, glycosylated hemoglobin, hemorheological indexes and blood coagulation.Results The level of blood viscosity, plasma viscosity, red cell rigidity index, and red cell assembling index in DM and IGT group is higher with a positive correlation to HbA1c but red cell deformation index is lower with a negative correlation to HbA1c than control group, meanwhile, the level of plasma prothrombin time (PT), activated partial thromboplastin time (APTT), and thrombin time (TT) in DM and IGT group is lower but plasma ifbrinogen concentration is higher than control group, the statistical difference is significant.ConclusionHemorheological properties and blood coagulation abnormality is found in DM and IGT patients, which presents in a hypercoagulability state that may aggravate the chronic vascular disease of DM.
