急性早幼粒细胞白血病非变异型与变异型的形态学特征的初步研究

The Preliminary Study of Morphological Characteristics in the Patients of Acute Promyelocytic Leukemia （APL） with t（15;17） and Those with t（11;17） Variant Translocation

目的 探讨急性早幼粒细胞白血病（acute promyelocytic leukemia,APL）非变异型t（15;17）与t（11;17）等变异型的实验室特征的异同点.方法 选取苏州大学附属第一医院66例确诊为APL的患者为研究对象,按细胞遗传学及分子生物学特征分为t（15;17）非变异型组（n=59）及t（11;17）等变异型组（n=7）,对两组患者的发病特征、外周血常规、外周血分类、骨髓象及骨髓细胞形态学特征进行分析比较.结果 非变异型t（15;17）APL患者与t（11;17）等变异型APL患者的血象、骨髓象表现特征类似,但前者血小板计数更低,骨髓异常早幼粒细胞形态学特征更典型,差异有统计学意义（P〈0.05）.结论 变异型APL与非变异型APL相比,骨髓异常早幼粒细胞形态学表现特征存在差异,在细胞形态学诊断中需注意.

Objective To study the similarities and differences of the characteristic between the patients of acute promyelocytic leukemia （APL） with t（15;17） and those with t（11;17） variant translocation. Methods Clinical and laboratory data from 66 APL patients were analyzed and compared who were grouped into APL with t（15;17） （n=59） and APL with t（11;17） variant translocation （n=7） according to the results of cytogenetics/molecular biology in the First Affiliated Hospital of Soochow University from July 2009 to December 2015. Results The characteristic of peripheral blood and bone marrow cells in the patients of APL with t（15;17） were similar to those of the APL patients with t（11;17） variant translocation; but the platelet number was lower and the morphological characteristic of abnormal promyelocytes were more typical than those of the APL patients with t（11;17） variant translocation with statistical significance （P〈0.05）. Conclusion The APL patients with t（11;17） variant translocation have different morphological characteristics in the abnormal promyelocytes of the bone marrow as compared with those of APL with t（15;17）, which should be noted in the morphological diagnosis.
