乙酰半胱氨酸泡腾片联合布地奈德治疗特发性肺纤维化的临床疗效分析

Analysis of Clinical Curative Effect of Acetylcysteine Effervescent Tablets and Budesonide in Treatment of Idiopathic Fibrosis of the Lung

目的观察乙酰半胱氨酸泡腾片联合布地奈德治疗特发性肺纤维化的临床效果。方法选取该院呼吸科2014年1月—2016年5月住院治疗的IPF患者30例,随机分为两组,观察组给予氧驱动雾化吸入布地奈德混悬液1 mg/次,2次/d。并饮用乙酰半胱氨酸泡腾片600 mg/次,3次/d。对照组分次口服泼尼松0.5 mg/(kg·d),连用4周后减量至0.25 mg/(kg·d)。8周后对比两组临床疗效。结果观察组呼吸困难程度(t=1.52,P=0.017)、肺功能(TLC、FEV1/FVC改善分别为P=0.046,P=0.022)、血气水平(t=2.45,P=0.038)改善程度均较治疗组有所提高,差异有统计学意义(P<0.05),治疗过程中,观察组不良反应明显少于对照组。结论乙酰半胱氨酸泡腾片联合布地奈德治疗能改善患者呼吸困难临床症状,改善肺弥散功能,对纠正低氧血症方面有较好疗效。

Objective To observe the clinical curative effect of acetylcysteine effervescent tablets and budesonide in treatment of idiopathic fibrosis of the lung. Methods 30 cases of inpatients with IPF in our hospital from January 2014 to May 2016 were selected and randomly divided into two groups, the observation group adopted the oxygen atomizing inhalation of budesonide 1 mg once and twice per day, and drink the aeetylcysteine effervescent tablets 600 mg once and three times per day, the control group took the metacortandracin of 0.5 mg/（kg·d）orally by several times, and the dose decreased to 0.25mg/（kg·d） after 4 weeks in a row, and the clinical curative effect was compared after eight weeks. Results The dyspnea degree, lung function （TLC and FEV1/FVC improvement P=0.046, P=0.022）, blood gas level improvement degree （t=2.45,P=0.038） in the observation group increased compared with those in the treatment group, and the difference had statistical significance（P〈0.05）,in the treatment course, the adverse reaction in the observation group was obviously fewer than that in the control group. Conclusion The acetylcysteine effervescent tablets and budesonide can improve the dyspnea clinical symptoms, improve the pulmonary diffusion function and have a better curative effect on correcting the meionectic blood.
