118例原发性结外淋巴瘤临床病理分析

Analysis of Clinical Pathology of 118 Cases of Primary Extranodal Lymphoma

目的探究原发性结外淋巴瘤的临床和病理分析特征,为今后临床诊断与治疗提供可靠的经验。方法随机选取118例于2010年10月—2016年10月来该院就诊的经免疫组织化学和病理确诊的原发性结外淋巴瘤临床患者,对所有患者的临床资料进行回顾性分析。结果原发性结外淋巴瘤的常见首发部位为胃肠道,鼻腔,扁桃体,纵膈,皮肤,脾,睾丸,骨及软组织等,且原发性结外淋巴瘤的首发症状根据部位的不同有不同的临床表现,但无明显特异性。原发性结外淋巴瘤首发部位为胃肠道的临床表现多为腹部不适,腹痛,腹胀,腹泻,体重减轻,恶心等。原发性结外淋巴瘤首发部位为鼻腔多为发热,鼻塞,流涕等。原发性结外淋巴瘤首发部位为纵膈的表现为咳嗽,气短,面部肿胀。经过分析发现原发性结外淋巴瘤的体征表现多为原发器官肿大或局部肿块形成。结论原发性结外淋巴瘤的早期诊断和治疗对患者的预后有重要意义,应警惕原发性结外淋巴瘤,在诊治过程中应该动态观察患者病情,治疗效果欠佳的患者应及时进行病理组织学检查,给予病理组织学检查高度重视。

Objective To study the clinical pathology of 118 cases of primary extranodal lymphoma and provide reliable experience for the future clinical diagnosis and treatment. Methods 118 cases of clinical patients with primary extranodal lymphoma confmned by theimmunohistochemistry and pathology in our hospital ＂from October 2010 to 06tober 2016 were randomly seIected and the clinical data of all patients were retrospectively analyzed. Results The common primary site of primary extranodal lymphoma was gastrointestinal tract, nasal cavity, tonsil, mediastinum, skin, spleen, testicle, bone and soft tissue, and the primary symptoms of primary extmnodal lymphoma varied from different sites but had no obvious specificity, and the clinical manifestations of primary site in gastrointestinal tract were abdominal dis- comfort, abdominal pain, abdominal distension, diarrhea, weight reduction and nausea, and the clinical manifestations of primary site in cavum nasi were fever, nasal obstruction and running nose, and the clinical manifestations of primary site in mediastinum were cough, shortness of breath and facial swelling, and the analysis showed that the vital sign manifestations of primary extranodal lymphoma were mostly primary organ swelling or local mass formation. Conclusion The early diagnosis and treatment is of important significance to the prognosis of the primary extranodal lymphoma, and we should prevent the primary extranodal lymphoma, in the diagnosis and treatment course, we should dynamically observe the disease conlition of patients and the patients with bad treatment effect should conduct the pathologic histolo- gy examination in time and attach highly importance to it.
