摘要
目的探讨高压氧在原发性肾病综合征治疗中的应用价值及对患者肾功能的影响。方法选取2014年12月~2015年12月,本院收治的100例原发性肾病综合征患者。回顾性研究患者临床治疗资料,根据治疗方法的不同,将其分为试验组(n=53)与对照组(n=47)。对照组患者使用常规治疗方法进行治疗,而试验组中患者在上述治疗基础上使用高压氧进行治疗。比较两组患者的治疗效果,并观察上述两种治疗方法对患者肾功能的影响。结果 (1)治疗前,两组患者血脂水平无明显差别(P>0.05),而治疗后,试验组患者血脂相关指标值(CHOL、TG)均低于对照组(P<0.05);(2)试验组患者治疗总有效率明显高于对照组(94.33%vs 76.60%,P<0.05);(3)试验组患者并发症(血栓、上消化道出血、呕吐、栓塞等)发生率明显低于对照组(3.77%vs 23.40%,P<0.05);(4)治疗前,两组患者肾功能评价指标(血β2微球蛋白、Ig G、24 h尿蛋白定量、血总蛋白)无明显差别(P>0.05),而治疗后,试验组患者肾功能改善情况明显好于对照组(P<0.05)。结论高压氧治疗原发性肾病综合征能显著降低患者血脂水平,治疗安全有效,且能明显改善患者肾功能。该方法值得在临床上进一步推广和使用。
Objective To investigate the application value of hyperbaric oxygen in the process of primary nephrotic syndrome and the influence on renal function. Methods One hundred patients with primary nephrotic syndrome in our hospital from December 2014 to December 2015 were selected. According to the different treatment methods, they were divided into two groups : experimental group ( n = 53 ) and control group ( n = 47). Traditional treatment meth- od was used in the control group, and the patients were treated with hyperbaric oxygen on the basis of the traditional treatment in the experimental group. The treatment effect was compared with the indexes about renal function. Results (1)Before the treatment, the difference about the level of blood lipid was not significant between these groups ( P 〉 0.05 ) , and after treatment, the levels of blood lipid indexes (CHOL, TG) in the experimental group were lower than those of the control group (P 〈 0. 05 ) ; (2)The ratio of total treatment efficiency was higher in the experimental group compared with control group(94. 33% vs 76. 60% ) (P 〈0. 05 ) ; (3)The incidence of side effects (thrombus, upper gastrointestinal bleeding, vomiting, embolism, etc) in experimental group were lower than that of control group(3.77% vs 23.40% ) (P 〈0. 05) ; (4)Before the treatment, there were no difference about renal function (~32 micro albumin for blood, IgG, urinary protein in 24 h, total protein for blood) (P 〉 0.05 ), and after the treat- ment, the indexes stated above in the experimental group were better than the control group (P 〈 0. 05). Conclusion Hyperbaric oxygen used in the treatment of primary nephrotie syn- drome can significantly reduce the level of blood lipid for the patients, with more safety and ef- fectivity. And it also can improve the renal function significantly, The method is worthy of fur- ther promotion and use in clinical practice.
