沈氏达郁汤加减方联合艾司西酞普兰对肝癌介入术后抑郁患者生活质量的影响

Effect of ShenShi DaYu Decoction combined with escitalopram on the quality life of depression patients with hepatocellular carcinoma after interventional therapy

目的:观察沈氏达郁汤加减方联合艾司西酞普兰对肝癌动脉化疗栓塞术(TACE)后抑郁患者生活质量的影响,探索其临床疗效。方法:选择TACE后抑郁患者100例,随机分为治疗组和对照组,每组50例,对照组患者口服艾司西酞普兰片10mg/次,1次/d。治疗组患者在对照组的基础上加用沈氏达郁汤加减方。治疗6周后观察两组患者生活质量及汉密尔顿抑郁量表评分的变化。结果:治疗后治疗组患者在汉密尔顿抑郁量表评分、4个单一项目积分(失眠、食欲减退、呼吸困难、腹泻)、3个症状子量表积分(疲劳、疼痛、恶心呕吐)方面低于对照组,差异有显著性意义(P<0.05);在5个功能子量表积分(躯体、角色、认知、情绪和社会功能)、总体健康状况积分方面高于对照组,差异有显著性意义(P<0.05)。结论:沈氏达郁汤加减方联合艾司西酞普兰可以明显改善肝癌介入术后抑郁患者的抑郁状态,明显提高患者的生活质量,为中西医结合治疗该类患者提供了有效方法。

Objective ： To observe the effect of ShenShi DaYu Decoction Combined with escitalopram on the quality life of depression pa- tients with hepatoceUular carcinoma after interventional therapy, and to explore its clinical effectiveness. Methods： One hundred depression patients with hepatocellular carcinoma after interventional therapy were selected and randomly divided into treatment group and control group, 50 cases in each group. The control group were given oral escitalopram tablets 10mg/times, 1 times/d. The treatment group were given oral herbal treatment on the basis of the ShenShi DaYu Decoction . The quality of life and Hamilton Depression Scale score betwween two groups were observed after 6 weeks of treatment. Results： after treatment, the Hamilton Depression Scale score, 4 single project integration （ insom- nia, anorexia, dyspnea, diarrhea）, 3 in sub scale scores （fatigue, pain, nausea and vomiting） in treatment group were lower than the con- trol group （P 〈 0. 05 ） ; while 5 functions the sub scale scores （ physical, role, cognition, emotion and social function）, integral overall health status were higher than the control group, there were significant differences between （ P 〈 0.05 ）. Conclusion： ShenShi DaYu Decoc- tion Combined with eseitalopram can significantly improve the quality life and depression status in depression patients with hepatocellular car- cinoma after interventional therapy. It will provide effective treatment for the treatment of liver cancer with combination of TCM and Western medicine.
