呼吸道感染患儿病原学特点及血清肿瘤坏死因子和C-反应蛋白的诊断分析

Etiology of respiratory tract infections in children and the diagnostic application of tumor necrosis factor and C-reactive protein

目的研究呼吸道感染患儿病原学特点及血清肿瘤坏死因子(TNF-α)、C-反应蛋白(CRP)诊断运用,为临床治疗提供参考依据。方法选取医院2015年1月-2015年12月收治的544例呼吸道感染患儿,按照感染类型分为细菌感染组318例及非细菌感染组226例,并选择同期在医院健康检查的250名儿童为对照组,观察呼吸道感染患儿病原学特点及TNF-α、CRP的差异。结果感染组患儿共培养出446株病原菌,以革兰阴性菌为主,共281株占63.00%;细菌感染组血清中TNF-α及CRP水平高于非细菌感染组和对照组水平,差异有统计学意义(P<0.05)。结论导致患儿呼吸道感染的病原菌相对较多,且具有季节性特点,且细菌感染的患儿血清TNF-α及CRP水平显著升高,对诊疗具有重要指导意义。

OBJECTIVE To explore etiology of respiratory tract infection in children and the diagnostic application of tumor necrosis factor（TNF-α）and C-reactive protein（CRP）,so as to provide references for clinical treatment.METHODS A total of 544 children with respiratory tract infection admitted to our hospital from Jan.2015 to Dec.2015 were selected,and divided into bacterial infection group（n=318）and non-bacterial infection group（n=226）according to the infection types,and 250 children of health examation at the same period were selected as control group.The pathogenic characteristics of children with respiratory tract infection and the differences of TNF-αand CRP were observed.RESULTS Totally 446 strains of pathogens were cultured in children of infection group,which were mainly 281 strains of gram-negative bacteria,accounting for 63.00% .The levels of serum TNF-αand CRP in bacterial infection group were significantly higher than those in non-bacterial infection group（P〈0.05）.CONCLUSIONPathogens leading to respiratory tract infection in children are relatively more,and have seasonal characteristics,and the levels of serum TNF-αand CRP in children with bacterial infection are significantly higher,which are of great significance for diagnosis and treatment.
