腹腔镜联合胆道镜在治疗胆结石患者的临床效果分析

The clinical effect analysis of laparoscope combined with bile duct endoscope in the treatment of gallstones

目的探讨腹腔镜联合胆道镜治疗胆结石患者的临床效果。方法选取2014年11月至2016年3月进行诊治的114例胆结石患者,运用随机数字法分为两组,开腹组采取传统开腹手术治疗,腹腔镜+胆道镜组采取腹腔镜与胆道镜联合治疗,每组57例,应用SPSS 16.0统计学软件进行处理,术中术后计量资料用(x珋±s)表示,组间比较采用独立t检验;临床效果和并发症的发生率采用χ~2检验。以P<0.05差异有统计学意义。结果腹腔镜+胆道镜组的总有效率为54例(94.7%),开腹组的总有效率为47例(82.5%),差异有统计学意义(χ~2=4.226,P=0.040);与开腹组相比,腹腔镜+胆道镜组的手术时间、术中出血量显著降低,排气时间和住院时间显著减少(P<0.05);术后并发症的总发生率为4例(7.0%),明显低于开腹组12例(21.1%),差异有统计学意义(χ~2=4.653,P=0.031)。结论腹腔镜联合胆道镜治疗胆结石的临床效果较好,且术后并发症较少,有利于患者的早日康复。

Objective To investigate the clinical effect of laparoscope combined with bile duct endoscope in the treatment of gallstones. Methods 114 cases gallstones patients were collected from November 2014 to March 2016 in our hospital,according to the random numbers divided into open group57 cases）（and laparoscope combined bile duct endoscope group（57 cases）,the open group was given conventional open surgery,and the laparoscope combined bile duct endoscope group was treated by laparoscope combined with bile duct endoscope,compared the therapeutic effect and the incidence of complication after operation. Statistical analysis were performed by using SPSS 16. 0 software,intraoperative and postoperative measurement data were presented as and examined by using t test; therapeutic effect and the incidence of complication after operation were examined by chi square test. A P value of 0. 05 was considered as significant difference. Results The total effective rate of laparoscope combined bile duct endoscope group in the treatment of gallstones is 54 cases（94. 7%）,the total effective rate of open group in the treatment of gallstones is 47 cases（82. 5%）,with statistical significance（χ^2= 4. 226,P = 0. 040）;Compared with the open group,the operation time,blood loss of the operation of laparoscope combined bile duct endoscope group significantly decreased,the evacuation time and hospitalization time significantly shortened（P〈 0. 05）; The overall incidence of complication after operation of laparoscope combined bile duct endoscope group is 4 cases（7. 0%）,the overall incidence of complication after operation of open group is 12 cases（21. 1%）,with statistical significance（χ2= 4. 653,P = 0. 031）. Conclusion The clinical effect of laparoscope combined with bile duct endoscope in the treatment of gallstones is preferably,and less postoperative complications,and it is benefit to the recovery of diseases.
