MRI与CT对小肝癌诊断价值对比分析

Comparative Analysis of MRI and CT in Diagnosis of Small Hepatocellular Carcinoma

目的探讨MRI与CT诊断小肝癌的影像学特征,对比二者在小肝癌中的诊断价值。方法选取我院2013年1月-2015年3月收治的56例(64个)小肝癌患者作为研究对象,对所有患者进行16排螺旋CT扫描及MRI扫描,比较两种检查方法对小肝癌的诊断价值。结果 56例64个小肝细胞癌平扫呈高、等、低密度,增强扫描动脉期明显强化呈高密度;56例64个小肝细胞癌T1加权像呈高、低、等信号,T2加权像以高信号为主,增强扫描动脉期明显强化,而门脉期与延迟期则以低信号为主。CT平扫检出率明显低于MRI平扫检出率(χ2=4.005,P=0.045);CT增强扫描检出率与MRI比较,差异无统计学意义(χ2=0.100,P=0.753)。结论螺旋CT及MRI增强扫描对小肝癌的检出率均较高,临床可结合患者个人情况选择最佳检查方案。

Objective To investigate the imaging features of MRI and CT in the diagnosis of small hepatoceUular carcinoma, and to compare the diagnostic value of the two in the diagnosis of small hepatocellular carcinoma.Methods 56 cases （64） of patients with small hepatocellular carcinoma treated in our hospital from January 2013 to March 2015 were selected as the research object. The 16 slice spiral CT and MRI scanning were performed on all patients. The diagnostic value of the two methods was compared. Results 64 small hepatocellular carcinomas in 64 cases scanned by the 16 slice spiral CT showed high, medium and low density, and enhanced scanning arterial phase was significantly with high density. T1 weighted images of 64 small hepatocellular carcinomas in 56 cases showed high, low, and other signals, but T2 weighted images mostly showed high signal. Enhanced scanning arterial phase was significant strengthening while the portal phase and delayed phase was mainly low signal. The detection rate of CT scan was significantly lower than that of MRI scan（x^2=4.005, P=0.045）. The difference was not statistically significant （x^2=0.100, P=0.753）, compared the detection rate of enhanced CT scan with that of MRI. Conclusion The detection rate of spiral CT and MRI enhanced scan for small HCC is higher, and the best examination plan can be selected according to the individual patient＇s condition.
