老年非小细胞肺癌患者中外周血T细胞的表达与癌胚抗原表达的相关性

The correlation analysis between peripheral blood T cells expression and tumor antigen expression in the elderly patients with non small cell lung cancer

目的探讨老年非小细胞肺癌患者中外周血T细胞的表达与癌胚抗原(CEA)表达的相关性。方法采用前瞻性方法,2013年5月到2015年5月选择在我院经病理确诊的65例老年非小细胞肺癌患者作为观察组,同期选择在我院进行体检的健康老年人65例作为对照组,两组都进行外周血T细胞表达检测与血清CEA表达检测,同时进行相关性分析。结果观察组与对照组的血清CEA含量分别为34.45±14.19ng/mL和2.19±0.13ng/mL,观察组明显高于对照组(P<0.05)。观察组的总T细胞比例为2.53±2.19%,明显低于对照组的6.54±2.98%(P<0.05),同时观察组的CD3^+、CD3^+CD4^+、CD3^+CD8^+的比例也明显低于对照组(P<0.05)。Spearman秩相关分析结果显示老年非小细胞肺癌患者外周血总T细胞比例与血清CEA含量之间的表达呈现明显负线性相关(r=-0.569,P<0.05);Cox多因素分析结果显示总T细胞、CD3^+、CD3^+CD4^+的比例为影响CEA表达的独立影响因素(P<0.05)。结论老年非小细胞肺癌患者中外周血T细胞与CEA表达都呈现异常情况,外周血T细胞下降与CEA高表达可能具有一定相关性,为此检测外周血T细胞与CEA表达可评价患者疾病状况。

Objective To investigate the correlation analysis between peripheral blood T cells expression and tumor antigen（CEA）expression in the elderly patients with non small cell lung cancer.Methods Used a prospective method,From May 2013 to May 2015,65 elderly patients with non small cell lung cancer by pathological diagnosis in our hospital were selected as the observation group,and the medical examination of the health of the elderly 65 cases were selected at the same period as the control group,two groups were given the peripheral blood T cells expression detecting and serum CEA detection and correlation analysis.Results The serum CEA levels of the observation group and the control group were 34.45±14.19 ng/mL and 2.19±0.13 ng/mL,the observation group was significantly higher than that of the control group（P〈0.05）.The total T cell ratio in the observation group was 2.53±2.19%,which was significantly lower than that of the control group of 6.54±2.98%（P〈0.05）,and the ratios of CD3＋,CD3＋CD4＋and CD3＋CD8＋ in the observation group were significantly lower than that in the control group（P〈0.05）.Spearman rank correlation analysis showed that the peripheral blood T cell ratio and serum CEA levels in the elderly patients with non small cell lung cancer patients were showed a significantly negative linear correlation（r=0.569,P〈0.05）.Cox multivariate analysis showed that the ratios of total T cells,CD3＋,CD3＋CD4＋ expression were the independent influence factors for the CEA levels（P〈0.05）.Conclusion The peripheral blood T cells and CEA expression in the elderly patients with non small cell lung cancer patients are showed abnormal situation,the decrease of peripheral blood T cells and the high expression of CEA may have some correlation,lfor the detection of peripheral blood T cells and CEA expression can evaluate the status of patients.
