摘要
目的探讨乳腺癌患者血清癌胚抗原(CEA)、Ki67及缺氧诱导因子1α(HIF-1α)水平与临床病理参数的相关性。方法选取2011年1月至2016年1月间东莞市人民医院收治的70例乳腺癌患者,采用免疫组织化学染色分析患者的HIF-1α和Ki67的水平,采用化学发光免疫分析法检测CEA水平,分析三种检验指标与患者临床病理参数之间的相关性。结果 40岁以下患者的CEA阳性表达率低于40岁以上患者,差异有统计学意义(P<0.05),CEA与肿瘤体积、TNM分期及组织学分期均无明显相关性,差异无统计学意义(P>0.05)。TNM分期≥Ⅱ期及组织学分期≥Ⅱ期患者的Ki67及HIF-1α水平呈高阳性表达的特点,差异有统计学意义(P<0.05),Ki67及HIF-1α水平与年龄和肿瘤体积并无明显相关,差异无统计学意义(P>0.05)。相关性分析结果显示,CEA与Ki67和HIF-1α均无明显相关性,差异无统计学意义(r=-0.194,-0.179,均P>0.05),Ki67与HIF-1α水平呈正相关,差异有统计学意义(r=0.475,P<0.05)。结论 Ki67和HIF-1α水平能较好地反映出乳腺癌患者的病情进展和预后情况,且两者具有相关性,可作为乳腺癌的有效标志物进行联合检测。
Objective To investigate the correlation of serum CEA, Ki67 and HIF-la levels with clinicopathologic parameters in patients with breast cancer. Methods Seventy patients with breast cancer who were treated at Donggnan People's Hospital from January 2011 to January 2016 were studied. The levels of hypoxia-inducible factor la (HIF-lct) and Ki67 were detected by immunohistochemical staining. Chemi- luminescence immunoassay was used to measure levels of carcinoembryonic antigen (CEA). The correlation between the three measures and clinicopathological parameters was analyzed. Results Positive expression rate was lower in patients less than 40 years than patients more than 40 years ( P 〈 0. 05 ). There was no significant correlation between CEA and tumor volume, TNM stage and histological stage ( P 〉 0. 05 ). How- ever, levels of Ki67 and HIF-la were higher in patients with TNM stage≥Ⅱand patients with histological stage ≥Ⅱ ( P 〈 0.05 ). The correlation analysis showed that no obvious relation was noted between CEA and Ki67 and HIF-Iot (r = -0. 194, -0. 179, all P 〉0. 05). There was a significant positive correlation be- tween Ki67 and HIF-la(r =0.475, P 〈0.05). Conclusion Compared with CEA, Ki67 and HIF-la levels can better predict development and prognosis of breast cancer. They have good correlation and can be used as an effective marker for breast cancer detection.
