尊严疗法在中晚期肺癌患者临终护理中的应用

Application of dignity therapy in hospice nursing for patients with advanced lung cancer

目的探讨尊严疗法在中晚期肺癌患者临终护理中的效果。方法选取2013年12月至2015年12月间青岛市市立医院收治的148例中晚期肺癌患者,采用随机数表法分为观察组与对照组,每组74例。观察组患者采用尊严疗法,对照组患者采用常规护理,比较两组患者护理前后自尊改变、希望水平和心理状况。结果两组患者护理后状态自尊量表(SSES)评分(行为自尊、社会自尊、外表自尊和总分)、希望水平评分(积极态度、行为态度、亲密关系和总分)较护理前增加,差异均有统计学意义(均P<0.05)。两组患者护理后症状自评量表(SCL-90)评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分均较护理前显著降低,差异均有统计学意义(均P<0.05)。观察组患者护理后SSES评分和希望水平评分均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者护理后SCL-90评分、SAS评分和SDS评分低于对照组,差异均有统计学意义(均P<0.05)。结论尊严疗法在中晚期肺癌患者临终护理中的疗效显著,可改善患者的自尊和希望水平,减轻心理负担。

Objective To investigate the efficacy of dignity therapy in hospice nursing for patients with advanced lung cancer. Methods A total of 148 patients with advanced lung cancer were selected at Qingdao Municipal Hospital from December 2013 to December 2015. Patients were divided into an observa- tion group and a control group by a random number table with 74 patients in each group. The observation group was given hospice nursing and the control group was given routine nursing. Changes in self-esteem, hope level and psychological status were compared between the two groups before and after nursing. Results After nursing, score of state self-esteem scale （SSES） （ behavioral self-esteem, social self-esteem, appear- ance self-esteem and overall score） and score of hope level （positive attitude, behavior attitude, intimacy and overall score） significantly increased in both groups compared with before nursing （P 〈 0. 05 ）. After nursing, scores of symptom checklist 90 （ SCL-90）, anxiety self-rating scale （SAS） and depression self-rating scale （SDS） significantly decreased in both groups compared with before nursing （P 〈 0. 05 ）. Scores of SSES and hope level were higher in the observation group than in the control group （ P 〈 0. 05 ）. Scores of SCL-90, SAS and SDS were lower in observation group than in the control group （ P 〈 0.05 ）. Conclusion Dignity thera- py shows significant efficacy in hospice nursing for patients with advanced lung cancer, which can improve patient＇s self-esteem and hope level and reduce psychological burden.
