锁骨钩钢板联合阔筋膜治疗陈旧性肩锁关节脱位39例疗效及近期随访研究

Curative effect and short term follow up of 39 patients with old dislocation of shoulder joint treated with clavicle hook plate combined with wide fascia

目的 探讨锁骨钩钢板与阔筋膜联合治疗陈旧性肩锁关节脱位39例的临床疗效.方法 选取2012年2月至2016年3月陈旧性肩锁关节脱位患者78例,依据手术方法不同分为对照组与研究组,每组39例.研究组进行锁骨钩钢板与阔筋膜联合治疗,对照组进行锁骨钩钢板治疗.统计两组临床效果、术后并发症发生情况,并对比手术前后肩关节功能及生活质量评分.结果 研究组（94.87%）优良率高于对照组（74.36%）,并发症发生率（2.56%） 低于对照组（20.51%）,差异有统计学意义（P〈0.05）.两组术前肩关节功能、生活质量评分比较差异未见统计学意义（P〉0.05）.与对照组相比,研究组术后半年肩关节功能、生活质量评分均明显较高,差异有统计学意义（P〈0.05）.结论 对陈旧性肩锁关节脱位患者予以锁骨构钢板与阔筋膜联合治疗效果较佳,能有效改善患者肩关节功能,提高其生活质量,安全性较高.

Objective To investigate the curative effect of clavicle hook plate combined with wide fascia on 39 cases of old dislocation of the shoulder.Methods From February 2012 to March 2016, 78 elderly patients with dislocation of the shoulder joint dislocation were divided into the control group and the study group according to the operation method, with 39 cases in each group.The study group was treated with the combination of the clavicle hook plate and the broad fascia, and the control group was treated with the treatment of the clavicle hook plate.The clinical effects and postoperative complications of the two groups were analyzed, and the function of the shoulder joint and the quality of life were compared before and after the operation.Results The excellent rate of the study group（94.87%） was higher than that of the control group（74.36%）, the incidence of complications （2.56%） was significantly lower than that in the control group （20.51%）, the differences were significant（P〈0.05）.There was no significant difference in the function or quality of life between the two groups （P〉0.05）.Compared with the control group, the study group had significantly higher scores of shoulder function and quality of life in the first half year after surgery, and the differences were significant（P〈0.05）.Conclusions Clavicular steel plate and fascia lata is effective on the old acromioclavicular joint dislocation, which can improve the function of the shoulder joint effectively.And it can improve their quality of life, and the security is high.
