摘要
目的 探讨肺表面活性蛋白(SP)和肺腺癌相关抗原(KL-6)在评估小儿支原体肺炎肺组织损伤中的意义.方法 选取99例小儿支原体肺炎患儿,另选因支气管异物入院的30例患儿作为对照组,利用ELISA法检测血清SP-A、SP-D和KL-6含量,所有支原体肺炎患儿行支气管肺泡灌洗术,对肺泡灌洗液中SP-A、SP-D和KL-6含量进行检测.结果 支原体肺炎患儿血清SP-A、SP-D和KL-6含量分别为(41.2±12.9)、(109.5±45.2)和(33.5±13.3)mg/L,均高于对照组,差异有统计学意义(t=14.921、10.111、10.838, P〈0.05);急性期患儿血清SP-A、SP-D和KL-6含量均高于恢复期患儿和对照组,且恢复期患儿高于对照组,差异均有统计学意义(P〈0.05);急性期患儿肺泡灌洗液中SP-A、SP-D和KL-6含量均高于恢复期患儿和对照组,且恢复期患儿高于对照组,差异均有统计学意义(P〈0.05);Pearson相关分析显示,支原体肺炎患儿血清中SP-D含量与肺泡灌洗液中SP-D含量呈正相关(r=0.394,P〈0.05),血清中KL-6含量与肺泡灌洗液中KL-6含量呈正相关(r=0.497,P〈0.05).结论 支原体肺炎患儿血清和肺泡灌洗液中SP-A、SP-D和KL-6含量均升高,血清SP-D和KL-6监测可作为评估支原体肺炎患儿肺组织损伤程度的指标.
Objective To investigate the changes and significances of pulmonary surfactant proteins and lung adenocarcinoma associated antigen in serum and bronchoalveolar lavage in children with mycoplasma pneumoniae.Methods Ninty-nine children with mycoplasma pneumoniae were selected.30 children undergoing bronchial foreign bodies were selected as the control group.The contents of SP-A, SP-D and KL-6 in serum were tested by ELISA method.The children with Mycoplasma pneumonia were taken bronchoalveolar lavage.The contents of SP-A, SP-D and KL-6 in bronchoalveolar lavage were detected respectively.Results The contents of serum SP-A, SP-D and KL-6 in Mycoplasma pneumonia were (41.2±12.9), (109.5±45.2) and (33.5±13.3) mg/L, which were higher than those in the control group, the differences were significant (t=14.921, 10.111, 10.838, P〈0.05).The contents of serum SP-A, SP-D and KL-6 in acute period of mycoplasma pneumonia were all higher than the recovery period and the control group, and the recovery period were all higher than those in the control group, the differences were statistically significant (P〈0.05).The levels of SP-A, SP-D and KL-6 in acute period of mycoplasma pneumonia were all higher than the recovery period and the control group, and the recovery period were all higher than the control group, the differences were statistically significant (P〈0.05).Pearson correlation analysis showed that the SP-D and KL-6 levels in the serum in children with mycoplasma pneumonia were positively correlated with those in bronchoalveolar lavage (r=0.394 and 0.497, P〈0.05).Conclusions The levels of SP-A, SP-D and KL-6 in serum and bronchoalveolar lavage fluid in children with Mycoplasma pneumonia were elevated.Monitoring the contents of SP-D and KL-6 in serum could be used as an indicator to assess the damage extent of lung tissue in children with Mycoplasma pneumoniae.
