腹腔镜下脾切除术在脾脏疾病中的应用及临床效果

The application and clinical effect of laparoscopic splenectomy in splenic diseases

目的分析与探讨将腹腔镜下脾切除术应用于脾脏疾病患者,研究其对应用价值及对临床疗效的影响及评价。方法将2014年5月~2016年5月期间经我院临床确诊后进行治疗的56例患者选择为临床研究病例,采用数字表法随机分成两组。对照组28例给予传统开腹手术治疗,观察组28例采用腹腔镜下脾切除术。比较两组术中出血量、排气时间、进食时间、疼痛评分、下床时间以及不良反应。结果观察组术中出血量(61.56±13.33)ml、排气时间(27.45±6.04)h、疼痛评分(3.87±1.91)分、进食时间(9.11±1.94)h、下床时间(6.47±1.54)h、不良反应(3.57)%均优于对照组术中出血量(102.44±30.47)ml、排气时间(39.36±6.81)h、疼痛评分(5.53±1.64)分、进食时间(46.69±3.67)h、下床时间(14.66±1.23)h、不良反应(28.57)%,差异有统计学意义(P<0.05)。结论脾脏疾病患者应用腹腔镜脾切除术治疗,减少术中出血量,降低手术风险,不良反应发生较少,临床治疗效果确切,有效促进患者机体各项生理功能恢复,有助于患者预后。

Objective To analyze and investigate the application of laparoscopic splenectomy in splenic disease patients,and to study the application value and clinical curative effect. Methods Fifty-six patients who were treated in our hospital from May 2014 to May 2016 were selected as the clinical cases,and they were randomly divided into two groups. 28 cases patients in the control group were treated by traditional open surgery,while the 28 cases patients in the observation group were given laparoscopic splenectomy. The amount of intraoperative bleeding,exhaust time,eating time,pain score,bed time and adverse reactions between the two groups were compared. Results The amount of intraoperative bleeding[（ 61. 56± 13. 33） ml],exhaust time[（ 27. 45 ± 6. 04） hs],pain score [（ 3. 87 ± 1. 91） points],bed time [（ 6. 47 ± 1. 54） hs] and adverse reaction rate（ 3. 57%） in the observation group were significantly better than the those in the control group,which was（ 102. 44 ± 30. 47） ml、（ 39. 36 ± 6. 81） hs、（ 5. 53 ± 1. 64） points、（ 46. 69 ± 3. 67） hs、（ 14. 66 ± 1. 23） hs、and28. 57%,respectively,and there were all difference between the two groups（ P 0. 05）. Conclusion The clinical application of laparoscopic splenectomy for splenic diseases patients can reduce bleeding,reduce the risk of surgery,less the adverse reaction,and can promote the body physiological function recovery of patients,it is helpful for patients prognosis.
