外周血中性粒细胞绝对值/淋巴细胞绝对值比值在急性百草枯中毒早期预后评估中的价值 被引量:11

摘要 目的探讨外周血中性粒细胞绝对值/淋巴细胞绝对值比值(NLR)在急性百草枯中毒早期评估预后的价值。方法选择2014年1月—2015年10月第四军医大学西京医院收治的百草枯中毒患者164例,根据随访结果分为死亡组和生存组,观察影响急性百草枯中毒死亡的危险因素,评价NLR对急性百草枯中毒预后的评估价值。结果164例患者存活65例,死亡99例,病死率60.37%。死亡组血白细胞计数、中性粒细胞、NLR高于存活组,血淋巴细胞、血钾低于存活组(P<0.05)。血淋巴细胞、NLR是百草枯中毒患者死亡的独立危险因素,NLR>12.07时预测百草枯中毒患者30 d病死率的特异度为96.9%,敏感性为65.7%。结论血NLR对百草枯中毒的预后有较高的评估价值。 Objective To investigate absolute neutrophil count to absolute lymphocyte count ratio in peripheral blood in early evaluation prognosis of patients with acute paraquat poisioning. Methods A total of 164 patients with pa- raquat poisoning admitted during January 2014 and October 2015 were divided into death group and survival group according to follow-up results. Risk factors affecting death of paraquat poisioning were observed, and value of absolute neutrophil count to absolute lymphocyte count ratio in evaluation of prognosis was analyzed. Results Among the 164 patients, 99 patients died, and the fatality rate was 60.37%. In dead group, values of white blood cell count, neutrophil count and absolute neutrophil count to absolute lymphocyte count ratio in blood were significantly higher, while values of blood lymphocyte and potassium were significantly lower than those in survival group ( P 〈 0.05 ). Lymphocytes and absolute neutrophil count to absolute lymphocyte count ratio were independent risk factors of death in patients with paraquat poisoning. When absolute neutrophil count to absolute lymphocyte count ratio was more than 12.07, the specificity and sensitivity in prediction of 30-day fatality rate of patients with paraquat poisoning were 96.9% and 65.7% respectively. Conclusion Absolute neutrophil count to absolute lymphocyte count ratio has a certain value in evaluation of prognosis of patients with paraquat poisoning.
