低位直肠癌保肛术后吻合口瘘的防治体会

Prevention and Treatment of Anastomotic Leakage after Anus-preserving Operation for Low Rectal Cancer

目的探讨低位直肠癌保肛术后对吻合口瘘的防治体会。方法选取本院2010年2月-2015年1月收治的行低位直肠癌保肛术患者80例,回顾性分析所有患者的临床资料。结果吻合口瘘者发生率为8.76%(7/80),平均发生时间为(6.4±1.6)d。1例行急诊横结肠造口,6例行保守治疗,5例患者痊愈,占81.20%。结论发现吻合口瘘后需要及早进行有效治疗,尽可能排尽肠内容残留物,对于吻合口瘘较大者或非手术无效者可进行第二次手术。

Objective To explore the prevention and treatment of anastomotic leakage after anus-preserving operation for low rectal cancer. Methods Eighty patients with preserved rectal cancer who underwent rectal cancer anesthesia from December 2011 to November 2016 were retrospectively analyzed and the clinical data of a/1 patients were retrospectively analyzed. Results The incidence ofanastomotic leakage was 8.76%（7/80）, the average time of occurenee was （6.4 ± 1.6） d. 1 case of emergency transverse colostomy, 6 cases of conservative treatment, 5 patients recovered, accounting for 81.20%. Conclusion The anastomotic leakage after the need for early effective treatment, as far as possible row of intestinal content residues, the larger anastomotic leakage or non-surgical failure can be a second operation.
