辅助呼吸训练合腹部按摩护理对慢性阻塞性肺疾病稳定期便秘患者的疗效

Efficacy of assisted breathing training combined with abdominal massage care in management of constipation in patients with chronic obstructive pulmonary disease

目的观察辅助呼吸训练合腹部按摩护理对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)稳定期便秘患者的疗效分析.方法将120例COPD稳定期便秘患者按照住院先后顺序编号,采用随机数字法分为对照组和观察组各60例.对照组给予常规护理;观察组在对照组治疗基础上给予辅助呼吸训练合腹部按摩护理,均连续干预2 wk.观察2组患者的临床疗效、便秘情况和护理满意率.结果护理干预后观察组有效率明显高于对照组(96.67%vs 83.33%),差异有统计学意义(P<0.05).干预前2组排便困难、粪便性状、排便时间、下坠、不尽、胀感、排便频度、腹胀等症状积分比较,差异均无统计学意义(P>0.05).干预后2组各临床症状评分均明显降低,差异有统计学意义(P<0.05);且观察组各症状评分明显低于对照组(P<0.05).对照组满意率与观察组比较,差异有统计学意义(66.67%vs 93.33%,P<0.05).结论辅助呼吸训练合腹部按摩护理能改善COPD稳定期便秘患者的临床疗效、便秘症状和提高护理满意率.

AIM To observe the efficacy of assisted breathingtraining combined with abdominal massage care in the management of constipation in patients with chronic obstructive pulmonary disease（COPD）.METHODS One hundred and twenty patients with COPD and constipation were randomly divided into either a control group or an observation group（n = 60 each）. Both groups were given conventional nursing care, and the observation group was additionally given assisted breathing training and abdominal massage therapy. After 2 wk, clinical efficacy,constipation and nursing satisfaction were compared between the two groups.RESULTS The effective rate was significantly higher in the observation group than in the control group（96.67% vs 83.33%, P 0.05）. Before intervention, the scores of symptoms such as difficulty in defecation, stool properties,defecation time and frequency, abdominal distension and other symptoms did not differ statistically between the two groups（P 0.05）. After intervention, the scores of the above clinical symptoms were significantly improved in both groups（P 0.05）, and the improvement was significantly better in the observed group than in the control group（P 0.05）. The satisfaction rate was significantly higher in the observation group than in the control group（93.33% vs 66.67%, P 0.05）.CONCLUSION Assisted breathing training combined withabdominal massage care can improve the clinical curative effect on constipation in patients with COPD.
