个性化康复辅导干预模式联合常规治疗对重症肺炎患儿心理状态和治疗依从性的影响

The impact of individualized rehabilitation counseling intervention combined with conventional treatment on the psychological state and the treatment compliance in children with severe pneumonia

目的探讨个性化康复辅导干预模式联合常规治疗对重症肺炎患儿心理状态和治疗依从性的影响。方法选取2013年12月至2015年12月期间在我院就诊的80例重症肺炎患儿作为研究对象,并分为观察组(n=40)和对照组(n=40)。对照组患儿给予常规的对症治疗措施,观察组患儿则采用个性化康复辅导干预模式联合常规对症治疗进行干预。在治疗前后采用儿童焦虑障碍自评量表(SCARED)和儿童抑郁障碍自评量表(DSRSC)对两组患儿的心理状态进行客观评定及比较,同时对两组患儿的治疗依从性以及家属对医疗工作的满意度进行比较。结果与干预前相比较,干预后两组患儿的SCARED和DSRSC评分明显较低(P<0.05),且观察组患儿的SCARED和DSRSC评分显著低于对照组(P<0.05);干预后,两组患儿的治疗依从性与干预前相比均有显著提高(P<0.05),且观察组患儿的治疗依从性显著高于对照组(P<0.05);观察组患儿家属对医疗工作的满意度明显优于对照组(P<0.05)。结论个性化康复辅导干预模式联合常规治疗可有效改善患儿的不良心理状态,提高患儿的治疗依从性,值得临床中推广实施。

Objective To explore the impact of individualized rehabilitation counseling intervention combined with conventional treatment on the psychological state and treatment compliance in children with severe pneumonia. Methods 80 children with severe pneumonia admitted in our hospital during December 2013 to December 2015 were selected as research subjects,and were divided into the observation group（n=40）and the control group（n=40）. Children in the control group were given conventional symptomatic treatment,and children in the observation group were offered individualized rehabilitation counseling intervention on the basis of the conventional treatment. The scores of Screen for Child Anxiety Related Disorders（SCARED）and Depression Self-rating Scale for Children（DSRSC）were compared before and after treatment between the two groups. The treatment compliance and their satisfaction degree to the treatment were also evaluated and compared between the two groups. Results After all intervention,children in both groups had statistically lower DSRSC and SCARED scores than before（P〈0.05）,and children in the observation group had significantly lower scores than those in the control group（P〈0.05）. In terms of treatment compliance,children in both groups had significant improvement after treatment（P〈0.05）,and children in the observation groups had higher treatment compliance than the control group（P〈0.05）. Families of children in the observation group had statistically higher satisfaction degree than those of the control group（P〈0.05）. Conclusion Individualized rehabilitation counseling intervention combined with conventional treatment is effective in improving the mental state and treatment compliance of children with severe pneumonia,and thus it is worth clinical promotion.
