循证心理护理对CPAP治疗COPD伴HF患者的负性情绪,心肺功能及生活质量的疗效

Effect of evidence-based psychological nursing on negative emotion,cardiopulmonary function and life quality during non-invasive CPAP in the treatment of patients with chronic obstructive pulmonary disease complicated with heart failure

目的探讨循证心理护理在无创持续气道正压通气(CPAP)治疗慢性阻塞性肺疾病(COPD)伴心力衰竭(HF)患者中的应用效果。方法将82例经无创CPAP治疗的COPD伴HF患者随机分为观察组与对照组,各41例。对照组实施常规护理,观察组在对照组基础上实施循证心理护理。比较两组心肺功能指标、治疗依从性及心理状态。结果护理后观察组的治疗依从率为92.68%,显著高于对照组的78.05%(P<0.05);护理后观察组的焦虑抑郁情绪和生活质量、心肺功能指标的改善程度均显著高于对照组(P<0.05)。结论循证心理护理应用于无创CPAP治疗的COPD伴HF患者能够改善不良情绪、改善心肺功能并提高治疗依从性。

Objective To evaluate the effect of evidence-based psychological on negative emotion,cardiopulmonary function and life quality nursing during non-invasive CPAP in the treatment of patients with chronic obstructive pulmonary disease（COPD）complicated with heart failure（HF）. Methods 82 cases of COPD patients complicated with HF who treated with noninvasive CPAP were randomly divided into the observation group and the control group（each for 41 cases）. The control group received routine nursing and psychological intervention,while the observation group received evidence-based psychological nursing on the basis of the control group. The cardiopulmonary function indexes,treatment compliance and mental state between the two groups were compared. Results The treatment compliance rate in observation group was significantly higher than the control group（92.68% vs 78.05%,P〈0.05）;after nursing,the improvement of anxiety and depression mood,quality of life and cardiopulmonary function indexes in the observation group were significantly better than those in the control group（P〈0.05）. Conclusion Evidence-based psychological nursing applied in non-invasive positive pressure ventilation in the treatment of patients with COPD complicated with HF has obvious promoting effect on improving patients＇ emotion accelerating the healing process,improving cardiopulmonary function and treatment compliance.
